Research On Rehabilitation Nursing Beds Admission And Discharge Criteria For The Oncology Patients

In line with the ageing of the population are aging, more people suffer and die from serious chronic disease(e.g. cancer) .Ddifficult medical services and expensive medical cost are protuberant problem in our country's current medical service system. According to the 11th Five-Year-Plan, in order to make the inpatients flow from general hospital, Shanghai Health Bureau devided the public hospital beds into several grades by different functional location, such as therapeutic beds, rehabiltation, nursing, and so on. To go out of the bottleneck and the development of a set of standardized referral criteria is a primary task.Data Resource: The studying subject is the sub-item of the research on rehabilitation nursing beds admission and discharge criteria for the patients which is supported by the Fund of Shanghai Health Bureau.Objective: Aim to develop the rehabilitation nursing beds admission and discharge criteria for oncology patients, and make clear the functional location of the different beds to aid the referral between them, and finally deal with the difficulty in seeking clinic service and admission for oncology patients.Methods: Based on the profound analysis of the center concepts and a large number of domestic and foreign literature review, semi-structured with open-ended questions interviews were undertaken with 15 professional who were recruited using purposive sampling and random stratified samping. The phenomenological analysis method was used to explore the existing admission and discharge criteria for oncology patients requiring rehabilitation nursing care and obtain expert opinion to find the direction and the dimensions of criteria. Then applying the theory of system to initially construct the main contents, and two Guideline draft involve medical and surgical oncology patients were framed. A modified Delphi method was used to collect the responses from 20 medical oncology and 36 surgical oncology experts in Shanghai by letters twice. The results were statistically analysed by using qualitative method, comprehensive scoring method and analytic hierarchy process(AHP).Result: (1)There are currently no standardized rehabilitation nursing beds admission and discharge criteria for the oncology patients in Shanghai. (2)The functional location of the rehabilitation nursing beds for oncology patients has been defined as followed:â‘ Treatment for the cancer patients with complex symptoms that are unresolved and the side effects of medication;â‘¡Continue treatment for the cancer patients with effective anti-tumor treatment protocols;â‘¢Palliative therapy and terminal care;â‘£Preoperative and postoperative rehabilitation. (3)A set of sandardized admission and discharge criteria including a programmatic general principle and specific criteria for several kinds of the tumor. (4)Make disscusses and suggestions.
