| Speciality:clinical combination of TCM with Western medicineAuthor:Deng YanyanTutor:proferssor Yi TiegangObjective:Renal hypertension due to renal disease is the secondary hypertension in the most common.A variety of renal parenchymal disease and decreased renal function disease may be associated with high blood pressure, and hypertension is the independent risk in the development of chronic kidney disease to the final stage of renal disease(ESRD).At present,the clinical treatment of renal hypertension based mainly on Western medicine,although its curative effect is precise,but the side effects can not be neglected. Recently,researches of traditional Chinese medicine on renal hypertension has put on the agenda,we have found that the well-known prescription of Chinese medicine Tian Ma Gou Teng Yin can not olny lower the blood pressure stablely, but al so maintain the yin-yang balance,improve clinical symptoms,and it price cheaper,less side effects in our preliminary research.But there is no theoretical basis on its depressurization and renal protective.This research measuring the plasma 6-Keto-PGF1a and ET concentration in patients with renal hypertension before and after the treatment of Tian Ma Gou Teng Yin,research its depressurization mechanism and advantages to provid an objective basis of Chinese medicine on renal hypertension from the perspective of modern medicine for the clinical application of better guidance,and the average blood pressure,24 urine protein,renal function,changes in clinical symptoms also be observed.Methods:In this research we select patients of renal hypertension,disease of Chinese medicine as "Vertigo" or "headache",Differentiation for hyperactivity type.28 cases of a total sample was divided into treatment group and control group adopting randomization design,each 14 cases.Two groups of patients were low-salt diet,less than 3g sodium intook daily.Attention to rest,avoid fluctuations in blood pressure such as emotion.Give the control group valsartan(Diovan) 80mg,everyday morning per os.The treatment group take Tian Ma Gou Teng Yin Decoction of origin prescription based on the control group.Each group accept a treatment of eight weeks.The plasma 6-Keto-PGF1a, ET concentration of each group is measured before and after the treatment,also, the average blood pressure,urinary protein of twenty-four hours,renal function,clinical symptoms are all observed,then made the statistical analysis to have a conclusion,and the research is completed.Results:From the data we found:there is no statistical difference between two groups before the treatment(p＞0.05).The plasma 6-Keto-PGF1a concentration of two groups is both higher by he treatment,and the statistical difference is significant(P＜0.05).After the treatment,there is significant statistical difference between two groups(P＜0.05).The plasma ET concentration of two groups is both lower by the treatment,and there is no statistical significant difference in control group(P＞0.05),but the difference in treatment group is statistical significant(P<0.05).After the treatment,there is statistical significant difference between two groups(P＜0.05).The average blood pressure is both lower by the treatment,and the statistical difference is significant(P＜0.05).After the treatment,there is statistical significant difference between two groups(P＜0.05).The 24-hour urine protein is both lower by treatment,and the statistical difference is significant(P＜0.05),but there is no statistical difference between two groups after the treatment(P＞0.05).Two groups of the serum Cr,BUN,SystatinC are all lower by the treatment,but there is no statistical significant difference in control group(P＞0.05).In treatment group,there is no statistical significant difference on the serum Cr,BUN(P＞0.05),but the statistical difference is significanton on the serum SystatinC of this group (P＜0.05).after the treatment,there is statistical significant difference on the serum SystatinC between two groups(P＜0.05).For Cumulative score of clinical symptoms,there is no statistical significant difference in control group(P＞0.05),but the difference in treatment group is statistical significant(P＜0.05)Conclusion:Chinese medicine of Tian Ma Gou Teng Yin can maintain in patients with renal hypertension the plasma concentration level of prostacyclin,and reduce the plasma concentration of endothelin,which sustain a stable blood pressure.It also can reduce the urinary protein,protect the renal function, delay the progress of chronic kidney disease and eliminate or significantly improve in patients with dizziness,headache,symptoms of waist soreness and legs weakness to improve the quality of life of kidney patients without evident vice-reaction.So,maybe it is worthing operating in clinical treatment.
