Effect Observation Of Tian Ma Gou Teng Yin Acupoint Application On Essential Hypertension With Hyperactivity Of Liver Yang

ObjectiveTo observe the effects of acupoint application in hypertensive patients with hyperactivity of liver yang syndrome,so as to reduce the patients’blood pressure,improve the traditional Chinese medicine syndrome (TCM syndrome),the quality of life (QOL),and reduce the level of Endothelin-I (ET-I),then provide a convenient and simple home-basic care programme for such patients.Methods74 objects were recruited according to the diagnosis,reception and exclusion criterion during March 2014 to September 2014 in cardiovascular internal medicine clinic,and they were randomly divided into test group (37 cases) and control group (37 cases).All objects received routine treatment and nursing,while the objects in test group received acupoint application therapy with Shenque and Yongquan (both sides) for 4 weeks,once per day,five times per week (from Monday to Friday) and eight hours every time.We observed and recorded the blood pressure,TCM syndrome and QOL before and after the acupoint application therapy as well as 4 weeks over the trial,and ET-I before and after the acupoint application therapy,then make comparisons after all the data were recorded.Results1. Baseline ComparisonThere was no significant difference in general condition (age,sex and educational level,etc) and basic clinical data (course of disease,medication and complication,etc) between the two groups before the trial (P>0.05).And there was no significant difference in SBP, DBP, pulse pressure,TCM syndrome,QOL and ET-I (P>0.05).And during the trial,the difference of the anti-hypertensive agent use every week between the two groups were not statistically significant (.P>0.05).It showed that the two groups were comparable.2. Effect of Blood PressureAfter 4 weeks of the trial,both SBP and DBP in test group had significantly decreased,the difference was statistically significant (P<0.01),while there was no statistical significant difference in control group (P>0.05).And compared with the control group,there was a significant difference in SBP (P<0.05).After 4 weeks over the trial,the SBP and DBP in both groups had decreased significantly compared with the beginning of trial (P<0.01),but there was no significant difference between the two groups (P>0.05).And compared with four weeks ago,the difference of SBP and DBP in both groups were not statistically significant (P>0.05).In addition,the graphical chart of SBP and DBP in both groups showed that the decline of blood pressure in test group was stabler compared with the control group.3. Effect of TCM syndromeComparison within group:After 4 weeks of the trial and after 4 weeks over the trial,the global sum of TCM syndrome,dizziness,headache,dry mouth and bitter taste in test group had a significant difference (P<0.01),while significant changes in the global sum of TCM syndrome were found in control group after 4 weeks of the trial (P<0.05).And after 4 weeks over the trial,the global surn of TCM syndrome,dizziness,headache and bitter taste in control group were significantly lower than that before the trial (P<0.01/P<0.05).Comparison between groups:After 4 weeks of the trial,there was a significant difference in the global sum of TCM syndrome,dizziness,dry mouth and bitter taste between the two groups (P<0.01//><0.05),while the difference of the global sum of TCM syndrome and bitter taste between the two groups were statistically significant after 4 weeks over the trial (P<0.01/P<0.05).After 4 weeks of the tial,the TCM syndrome effect in both groups showed that the total effective rate in test group was 83.3%,while the total effective rate in control group was 61.8%,the difference was statistically significant (P<0.05).4. Quality of Life (QOL)Comparison within group:After 4 weeks of the trial,the global sum of QOL, physical symptoms,somatization symptoms,sleep status,anxiety,depression,work status and hostility in test group had a significant difference (P<0.01/P<0.05),while significant changes in the global sum of QOL,physical symptoms and somatization symptoms were found in control group (P<0.01/P<0.05).After 4 weeks over the trial,the global sum of QOL in both groups had increased significantly compared with the beginning of trial (P<0.05).Comparison between groups:After 4 weeks of the trial,there was a significant difference in the global sum of QOL, physical symptoms, somatization symptoms, sleep status, anxiety, depression,work status and hostility between the two groups (P<0.01/P<0.05).And after 4 weeks over the trial,the difference of the global sum of QOL between the two groups were statistically significant (P<0.05).5. ET-IAfter 4 weeks of the trial,the level of ET-I in both groups had significantly decreased,the difference was statistically significant (P<0.01).But compared with the control group,there was no significant difference in ET-I (.P>0.05).Conclusion1. Acupoint application therapy can help reduce the SBP of hypertensive patients with hyperactivity of liver yang syndrome,and improve the stability of lowering blood pressure.2. Acupoint application therapy can ameliorate the TCM syndrome of hypertensive patients with hyperactivity of liver yang syndrome,namely dizziness,headache,dry mouth and bitter taste,and maintain the long-term efficacy.3. Acupoint application therapy can effectively improve the QOL of hypertensive patients with hyperactivity of liver yang syndrome,ameliorate the physical symptoms, somatization symptoms,sleep status and work status,and decrease anxiety, depression and hostility.4. Acupoint application therapy can decrease the level of ET-I of hypertensive patients with hyperactivity of liver yang syndrome,then improve the vascular endothelial function.
