| Purpose:(1)Through the analysis of the hospitalization and cost of the patients under the new rural cooperative medical care in the recent 3 years, the purpose of this paper is to understand the impact of the new rural cooperative medical care on medical expenses of participation farmers.(2) By analysing the case of the referral patients and the referral diseases in Jiutai city, it aims to provide reference on the building of medical institutions. The result of this paper offers effective measures to provide the basis for cost control on the new rural cooperative medical care, provides fundamental ruls for locating reasonable health resources, improving the quality of the health care, protecting the health of residents, improving the ratio of Participation, as well as meeting the demand for hospital inpatient services.Method:(1)Through the analysis based on the fundamental statistic information of various level of economy in city of Jutai, one methord of this paper is to unveil the economy level effects on participation, the cost of hospital and compensation.(2)The other method is to classify the disease of referral patients and the hospitals, exclude the case which admitted to hospital for the same first diagnosis of duplication, then classify the first diagnosis and the diagnosis of chronic diseases, and analyze of the basic situation of disease.(3)Based on the randomly selected data of human resource and instruments of People's Hospital in Jutai City, the author makes a analysis on basic construction of human resource and health institution .Statistic analysis:In this paper, the author will take qualitative and quantitative research to study the problem of the consrtuction on new rural cooperative medical system, collect the information through literature method, observation method and conversition. The entry of those data is abided by excel data analysis under strict supervision. Moreover, a statistic analysis method will be applied through SAS 8.02 software in this paper. The qualitative data will be analyzed by Chi-square test, and these quantitative data will be described by mean, median and quartile range, and be analyzed by Wilcoxon rank sum test as well.Result:(1)Since the new rural cooperative medical care has been carried out in Jiutai city, the work of the new rural cooperative medical care runs a smooth under the correct leadership of the business sector in the higher level of care and experimental work, under the instruction from the municipal party committee and government, the ratio of participation was steadily increased in 2006 -2008. There is great difference of participation according to the different economic level, along with the space spreads wider, the ratio of participation reduces gradually.(2)The ratio of hospital utilization is elevating since 2006 to 2008, despite the quantity of the patients in urban hospital is large, the spending on medical care is still low. In the contrary, there is a totally opposite situation in those provincial hospitals. As a result, it can be draw a conclusion that the ratio of the inpatient-out-location and related cost in towns is differs from each other largely according to the distance difference and economic difference.(3)The rate of inpatient-out-location is climbing from 2006 to 2008 with the increasing corresponding patient costs. The rate of inpatient-out-location is faliing as the decrease of economic level, and it climbs as the distance to the city of Jutai increases.(4)The hygiene manpower and technological resource as well as the equipment are relatively deficient in rural areas, the fundamental health instruments are not be fully adapted, and that circumstance leads to a consequences that most of the patients who are originally hospitalized in the urban hospital swamp into the provincial hospital, thence force the medical mechanism in different hospitals go out of control.Suggestion on policies:(1)Enhance the publicizing of medical cooperation to strengthen the confidence of the patients towards the cooperation policy.(2) Re-design the number, size and distribution of medical institutions in town according to geographical location, population distribution and other factors.(3)Adjusting the payment line, improving the proportion of subsidy and improving the referral system to guide the peasants to choose the proper medical institutions, use the grass-roots health resources and basic medical services.(4) Developing and fulfilling the relevant policies, then perfecting the qualities of facilities and services so as to increase the utilization of elementary medical instruments, thus applying the patient's need of medical care in rural area. At last we should expand the coverage of medical Insurance to avoid the poverty phenomenon caused by disease.
