The Current Situation And Suggestions About The New Rural Cooperative Medical System In Changchun City

Health status of rural residents has a significant impact on the overall health status of a society for its huge population. Moreover, it also determines the ability,economic development and modernization pace of a society. With the development of NCMS(the new rural cooperative medical system), the burden on rural participators who were engaged in serious illness has been well eased, for the reason of which,NCMS is now attracting more and more rural people to actively take part in.Strengthen NCMS, improving the level that rural people can benefit from NCMS and releasing the burden of medical costs of rural residents are always primary goals of rural medical system reforming.【Objective】 In this study, by evaluating elements such as the participation,fundraising and expenditures, expense reimbursement, referral levels and serious illness insurance, I have studied the operation status of NCMS of Changchun in 2014 from different aspects. Also, I objectively evaluated the achievements and analyzed the remaining problems of NCMS and the main factors that affect the rural patients`benefits from NCMS. Depending on those work, I propose targeted suggestions to try to complete NCMS.【 Methods 】 I start the study by Combining literature research and actual investigation. With the software of SPSS13.0, I statistically analysis the data of personal capita funding level, personal capita payment, participation rate, yield,average payment per capita net income of the proportion of fund utilization, the actual compensation ratio, inpatient hospital costs and both times the compensation ratio,which are all quoted from related materials about NCMS in Changchun, 2014.【Results】National financing contribute 43.9% of the fund rising of NCMS.Provincial and personal funds respectively take a share of 22.20% and 21.59%. Rural individuals of Changchun spent an average of 90 yuan on NCMS in 2014, which occupied 0.89% of rural personal annual income.Changchun government distributes NCMS fund under the guidance of Compensation Scheme of Jilin Province. Fund for hospitalization, which is the majorsource, make up 72.20% of NCMS fund. In 2014, 86.61% of the disbursement of NCMS fund is for inpatients and 13.39% for outpatients.Benefits of rural people that participate NCMS: 27.00% of outpatients successfully applied for reimbursement, which is higher than 9.3%—the percentage of inpatients who benefit from NCMS. Among outpatients who benefit, 68.71% of an average cost of 267.38 yuan can be reimbursed, and, 47.36% of an average of inpatient cost of 7,069.45 yuan is paid by NCMS. Ratio of reimbursement differs from the hospital levels: township: 71.38%, district: 61.91%, city: 40.76%, province:37.84%, out of province: 33.58%.Situation of transfer treatment between hospitals of different level:22.86% more in patients who had participated NCMS chose municipal hospitals in the first half of2014, after the implement of the transfer administration policy. Take the central hospital of Changchun as an example, there are 2,536 inpatients in the first half of2014, which is nearly twice than first half of 2013.Situation of insurance for serious illness: 28,287 person-time had benefited from the serious illness insurance. An average of 3,893.38 yuan can be reimbursed in each case. 43.57% of personal medical expenses are compensated before the insurance and54.25% after.【Conclusion】Rural individuals spend little but can benefit much in New rural cooperative medical care system. But the gap of hospital actual compensation ratio and policy compensation is bigger, need a further improve in the benefit level. After the implementation of a serious illness insurance system, although compensation level has increased, the farmers still have to pay almost half of the medical treatment charge.So farmers’ burden did not radically reduce. Since Changchun adopts a hierarchical referral system, in the change of the fixed-point hospital visitors have a better effect,whether the system is worth further to carry out, it also need to comprehensive evaluation. So, depending on the operation status of NCMS in Changchun, the government needs to take action to optimize the fundraising, fund distribution and payment, strengthen the service ability of grass-roots medical institutions to improve the efficiency of NCMS.
