Objectives To establish rural residents’health evaluation index system and evaluation methodology based on the current Disease Monitoring systems and New-rural Cooperative Medical System of rural areas; to evaluate the rural residents’health situation of Maiji district, Tianshui city according to the established evaluation index system and evaluation methodology, and screen the main public health issues of Maiji district, to provide scientific supports for local health service and policy making; and to explore the data cooperation of current diseases surveillance systems in order to utilize rural residents health evaluation efficiently.Methods Scientific and government literature on health evaluation and the disease monitoring systems were reviewed. The on-the-spot investigation of disease and health monitoring systems were conducted in Maiji district of Tianshui. Data of the Infectious Diseases Monitoring System, the Death Cause Monitoring System and the New-rural Cooperative Medical System were collected from Maiji CDC. The evaluation indexes system of the rural residents’health was established. The health situation of the local rural residents of Maiji was evaluated by established single evaluation indexes and then by complicated evaluation indexes. The single evaluation indexes of this study included indexes of incidence, mortality and admission rate, and the latter included life expectancy, potential years of life lost (YPLL), index of life lost, cause eliminated life expectation (CELE), potential value years of life lost (VYPLL), potential family years of life lost (FYPLL), per capita medical expenses. On using the analytic hierarchy process (AHP) method, the quantitative analysis was assigned with weights of indexes. The comprehensive grades index (G1) was gotten to evaluate comprehensively the health situation of the local rural resident and to screen the main public health issues affected local residents’health.Results1.The study result of single index analysis:the rank of the top five diseases are viral hepatitis, tuberculosis, influenza, bacterial dysentery, and infectious diarrhea, which have been used the incidence index and showed the impact of infectious diseases to local population; the rank of the top five diseases are cerebrovascular disease, ischemic heart disease, high blood pressure, emphysema, and stomach cancer, which were screened by the mortality index and showed the death strength caused by disease; the rank of the top five diseases were traffic accident, cerebrovascular disease, ischemic heart disease, suicide, and high blood pressure, which were screened by the YPLL index and showed years of life lost because of diseases; the rank of the top five diseases were cerebrovascular disease, ischemic heart disease, high blood pressure, traffic accident, and stomach cancer, which were screened by the CELE index and showed the decrease of life expectancy due to diseases; the rank of the top five diseases were birth asphyxia, premature birth, hemorrhagic disease of newborn, congenital pneumonia, and neonatal respiratory distress, which were screened by the index of life lost index and showed the average loss of life years resulted by diseases; the rank of the top five diseases were traffic accident, suicide, cerebrovascular disease, ischemic heart disease, and high blood pressure, which were screened by the FYPLL and showed the loss of family degree as a result of diseases,2. Analytic hierarchy process method was used to evaluate by the comprehensive grade and to screen the main public health issues of rural areas. The study result of comprehensive grade evaluation was traffic accidents, cerebral vascular disease, ischemic heart disease, hypertension, and stomach cancer.Conclusions The health evaluation index system of rural residents has been established in this study, which comprehensively analyzed single indexes to reflect the general health situation. The result showed which is an effective and comprehensive method to reflect the health situation of rural residents, and the main public health issues of rural areas can be screened efficiently. Traffic accidents, cerebral vascular disease, ischemic heart disease, hypertension, and stomach cancer were the main public health issues affected Maiji people’s health, and should be prevented and controlled prior by health policy-makers. It is needed to strengthen the sharing and data effective utilization of the different disease and health monitoring systems. So that the main public health issues of rural areas could be screened and comprehensively health situation of rural residents can be evaluated.
