The Clinical Orthogonal Design Of Acupuncture Therapy In Treating Nerve Root Cervical Spondilosis

Objective: To observe the influence of four factors on acupuncture effect, compare the effect of two levels of each factor and investigate if there is interaction between two factors by the Orthogonal design of four acupuncture factors in two levels, and finally select the best therapy about these four factors of acupuncture in treating nerve root cervical spondilosis. The four factors are increasing local points selection A, combining distal points selection along the meridian B, wave type of electroacupuncture C and alternation between two treatment D.Mehthods: First design orthogonal table L₈ (2⁷) of four factors in two levels. Sixty four patients of nerve root cervical spondilosis were divided into eight group randomly with eight patients in each group and were treated by therapies according to the orthogonal table L₈ (2⁷) . All patients were evaluated by VAS and CASCS. The results were treated by SPSS 13. 0 and analysed by direct analysis and variance analysis.Results: After the treatment, the results of VAS and CASCA show that factor A and factor B obtain significant statistical significance (P <0. 05) , the direct analysis shows K1 of both factor A and factor B. is greater than K2, which shows that increasing of local points and combining of distal points can improve antalgic effect and therapeutic effect; the results of VAS after first treatment show that there is statistical significance in factor C (P <0.05), the direct analysis shows that K1 is less than K2 which means dilatational wave is better than continuous wave in immediate antalgic effect; after two weeks, factor D obtains significant statistical significance in results of CASCS (P<0. 05), and K1 is less that K2 which means the acupuncture effect of treating once every other day is better than once every day. And the results of CASCS shows there is significant statistical significance in AB the interaction of factor A and factor B (P <0. 05), the comparison of K between different combination of BC shows the combination B1C2 is the best one, means the interaction between factor B and factor C have remarkable influence in acupuncture effect, the effect of combination B1C2 is much better that effect of combination B2C2 and B1C1 which means combination B1C2 can improve the effect distinctly.Conclusion:1. Increasing local points selection and combining of distal points can remarkably improve antalgic effect and therapeutic effect; dilatational wave is better than continuous wave in immediate antalgic effect; treatment once every other day is better than once every; the interaction between factor B and factor C has remarkable influence in the therapeutic effect, and the application of dilatational wave on distal points can improve acupuncture effect obviously.2. Treating nerve root cervical spondilosis by distal point is better than treating mainly by local points in reducing adverse reactions.3. The best therapy of four factors of acupuncture in treating nerve root cervical spondilosis is: A2B1C2D2, means the therapy that select distal point based on Jingjiaji, treated by dilatational wave and once every other has the best effect.
