Objective: Give a comprehensive assessment to perioperative psychological pressure and immune status. Understand the psychological stress,personality characteristics, the ability to cope and immune status in perioperative breast cancer patients deeply, analyze the psychological crisis about its nature, strength and main cause; discuss the changes in the level of The serum cortisol (Cor), interleukin-2 (IL-2) of perioperative breast cancer patients under psychological stress state with human-based psychological intervention; relativity of psychological stress and the immune function; relativity of various negative emotions and bad ways to the relevance of; the establishment of human-based psychological intervention models; provide a theoretical basis for clinical psychological intervention.Materials and Methods: This study is prospective randomized controlled clinical trials, 60 patients pathologically confirmed with breast cancer that come from breast surgery of the First Hospital of Jilin University are randomly divided into experimental and control groups, each have 30 cases. Make blood test of IL-2 and Cor changes level at 6:00 am after the No. 2 days and 10 days after surgery, and fill out a general questionnaire and SCL-90. Give a series of human-based psychological intervention on the experimental group, only routine care to the control group before and after surgery, without psychological intervention.Healthy control group has 30 patients, also undertaking blood test of Cor and IL-2 in the morning at 6:00, filling out a questionnaire in general and SCL-90, as a reference indicator. General questionnaire of three Groups match basicly.Results: 1. Stimulation from social factors, the history of mammary hyperplasia and family history of breast cancer in breast cancer group were significantly higher than those in healthy control group. It shows that breast cancer patients have more negative life events; patients with mammary gland hyperplasia history and family history of breast cancer have obvious mental stress, are in the state of chronic psychological stress, so that the crew in steady-state balance.2. Effects of Psychological intervention on the psychological status of patients with breast cancer2.1 Before surgery, SCL-90 total scores, total average scores, and positive projects, the average of positive symptoms and other factors in breast cancer group were higher than those in healthy control group, which means that the breast cancer patients are in a strong stress, having more psychological problems.2.2 After psychological intervention, the total scores, total average scores, and positive projects, the average of positive symptoms and other factors of breast cancer experimental group were significantly lower than that of sub-control group, expect mental factor. There was significant difference (P <0.001).2.3 Compare SCL-90 scores in itself before and after the surgery of breast cancer experimental group, the scores were obviously lower, except terrorist factor, with significant difference (P <0.001).2.4 In their own comparison of SCL-90 score before and after surgery of breast cancer control group, the total scores, total average scores, average positive symptoms, and scores of obsessive-compulsive symptoms, anxiety, hostility, paranoia, psychosis factor decreased, with significant difference (P <0.001); but there are no significant difference (P> 0.05)in the number of positive items, somatization, interpersonal relationships, terror, and scores of other factors. 3. Effect of Psychological intervention on immune function of patients with breast cancer3.1 In breast cancer group before surgery, the serum levels of Cor were significantly higher than those in healthy control group with significant statistical significance(P <0.01); IL-2 levels were significantly lower than those in the healthy control group with significant statistical significance (P < 0.01).3.2 In breast cancer experimental group after psychological intervention, Cor serum levels were significantly lower than the control group, with significant statistical significance (P <0.01); IL-2 test results were significantly higher, with significant statistical significance (P <0.01).4. Analysis on the correlation of SCL-90 score and immune function in breast cancer groupCor levels were positively correlated with the SCL-90 scores, but negatively correlated with IL-2. The higher factors points in SCL-90, the worse psychological situation, the more psychological problems, the lower their immune function.5. Through multiple factors correlation analysisCor and IL-2 showed a significant negative correlation (P <0.001), anxiety and depression were significantly correlated positively (P <0.001), depression with somatization, interpersonal sensitivity, hostility were significantly correlated positively (P <0.001); the anxiety somatization, interpersonal sensitivity, hostility were significantly correlated positively (P <0.001).Conclusion:1. Perioperative breast cancer patients have personality characteristics with neuroticism, emotional instability and the higher extent of covering up. There exists an obvious emotional disorders and unhealthy psychological conditions.2. Negative life events can lead to serious negative emotional reactions and poor coping style.3. Psychological stress of perioperative patients with breast cancer affects neuroendocrine immune system seriously.4. Breast cancer patients show a higher stress and a low immune status than in healthy control group.5. Human-based psychological intervention on perioperative breast cancer patients can reduce stress levels, improve mental health, and reduce negative coping style, increase response capacity and immune function.
