Analysis Of The Relationship Between The Assistant Check-up And The Differentiation Of Diseases And Symptoms In Nipple Discharge

Objective:To study the relationship between the differentiation of diseases and symptoms manifestation under breast adjutant scopy,and search for the impersonal index for differentiation of symptoms and signs. Methods:General data of 520 patients of nipple discharge were collected and analysed by differentiation of diseases and signs combined with B-ultrasound and breast fiberoptic ductoscopy. The routine examination of nipple discharge was applied to all the patients,and 68 of them were treated with pathologic examination. With statistical analysis, the relationship was studied between routine examination and differentiation of diseases and symptoms.The clinical diagnosis of nipple discharge should be supported with objective evidence from different angle. Accordingly,the sound objective index of differentiation of symptoms and signs should be construted.Results: (1)The age,history of fetation and breed of the patient, character and quantity of discharge have relations with the breast diease and differentiation of symptoms and signs . (2)The disease of nipple discharge correlate with differentiation of symptoms and signs:cyclomastopathy mostly show off stagnation of QI due to depression of the liver and chong-ren-disorder; ductal ectasia mostly show off stagnation of QI due to depression of the liver and stagnation of QI and stagnation heat; intracanalicular papilloma(disease) were syndrome of inability of the spleen to control blood flow and syndrome of up-flaring of the liver-fire; (3)The routine examination of nipple discharge correlate with differential diagnosis of diseases and differentiation of symptoms and signs.?The routine examination of nipple discharge: there is mainly fat drop in cyclomastopathy, mainly fat drop and white blood cells in ductal ectasia, red blood cells in intracanalicular papilloma, fat drop tagnation of QI due to depression of the liver and chong-ren-disorder,mainly red blood cells, inability of the spleen to control blood and Angiogenesis type. (2)The examnation with B-ultrasound:the percent of non-occupying lesion is 92.08%,in which the percent of cyclomastopathy is 83.22%,of which the image mainly display cystic hyperplasia,and the percent of ductal ectasia is 9.85%,of which mainly diplay lots of duct ectasia.The percent of occupying lesion is 7.92%,which mainly display as intracanalicular papilloma(disease). There was no significant difference between them.â‘¢fiberoptic ductoscopy: The percent of non-occupying lesion is 71.62%, the percent of occupying lesion is 28.38%.The detection rate of fiberoptic ductoscopy is greater than the B-ultrasound.The appearance of galactophore in cyclomastopathy mostly is normal,and the comedomastitis I type and galactophore rigidity and angusty II type are frequent.The ductal ectasia I type is the main appearance and galactophore rigidity and angusty is also can be found,which is maybe relative with the choice of cases.It is about 76.16% which can be found occupying pathological changes with microscope in Intracanalicular papilloma(disease), in which 2 examples was diagnosed conceivable case of cancer.The pathological change result confirm as early carcinoma.The pattern of syndrome in TCM is Angiogenesis type.During the 4.68 operational case, the detection rate and the rate of diagnosis of breast occupying case with fiberoptic is far larger than with the breast B-ultrasound.
