Objective:to observe the clinical effects of drug perfusion of houttuynia injection or gentami-cin injection on on non-neoplastic nipple discharge with Fiberoptic ductoscopy, and to explore the mechanism by observing the level of TNF-alpha and IL-6.Methods:1.120 cases of tandard patients were collected into this study according to Accepti-ng criteria, who were assigned randomly into two groups, the treatment group receiving houttuyniae injection infusion treatment, the control group receiving gentamicin injection infusion treatment.2. All patients were treated once a week, until the discharge stop. Observe the change of discharge amount were observed before and after treatment and TNF-alpha and IL-6 levels in discharge were evaluated by ELISA.3. The comparison between the results of quantitative data were performed by t test, qualitative data difference between groups were evaluated by χ2 test. Cure time difference is evaluated by the Log-rank test. Affecting factors of cure rate is analyzed with the Cox proportional hazards model. Inspection standard is 0.05, all the analysis are completed by SPSS17.0.Results:1. The cure rate was 94.9% in group treated with houttuynia versus 82.8% in gentam-ycin group (P<0.05) 2. The cure time is 3.67 weeks versus 4.24 weeks (P<0.05) between g-roups treated with houttuynia and gentamycin 3.The levels of IL-6 and TNF-alpha before and after treatment in two groups,the difference was statistically significant (P<0.05).The levels of IL-6 and TNF-alpha after treatment in houttuynia group are less than those in gentamycin group.4.There are no difference in the change of levels of IL-6 and TNF-alpha before and after treatment(P>0.05)between the two treatment groups.5. It is indicated by COX proportional hazards regression analysis that lower level of TNF-alpha is paralleled with higher cure rate while the level of IL-6 before treatment makes no contributes to the prognosis.The cure rate and median cure time in group subjected to Houttuynia injection treatment were superior to those in group teated with gentamicin injection.Conclusion:1. The non-neoplastic nipple discharge can be cured by perfusion treatment under Fiberoptic ductoscopy 2. Houttuynia injection treatment is superior to gentamicin injection 3. The mechanism of perfusion chemotherapy treatment of non-neoplastic nipple discharge with fiber laticifer microscopically may be related to the effection which the perfusion chemotherapy can reduce the discharge of IL-6 and the content of TNF-alpha.
