Background and objection:Lubar intervertebral disc protrusion (LIDP) is common disease. Backleg pain is main clinical manifestation of LIDP. According to modern medical research, the backleg pain of the symptoms caused by LIDH is the direct mechanical compression, of nerve fibers in the outer layers of the annulus preceded the rupture of the disk material into the spinal canal or the direct pressure on the nerve root. Conservative treatment can help to control and reduce the symptoms of LIDP. In this way, conservative treatment helps to avoid unnecessary surgery. According to the pathophysioloy of LIDH and the spinal biomechanics theory, three-dimensional oblique pulling method is formed. The characteristics of the method include a three dimensional motion manipulation, adjusting the posture of the lower limb; aim at increase the force of rotation on the targeted area and reduce the force on the unaffected area. The goal of three-dimensional oblique pulling method is to increase disk space and the intervertebral foramen space, to resolve the direct pressure on nerve root by the herniation, and finally resolve the symptoms that/is caused by LIDH.Method:157 volunteer LIDP patients were informed and consented. The volunteers were randomly separated into two groups: treatment group and control group. Volunteers in treatment group received the treatment using three-dimensional oblique pulling method; volunteers in control group received the treatment using traditional manipulation method. Volunteers received treatment once two days and totally seven treatments were preceded. The back pain level, lumbar motility, walking motility and the straight leg raising test were recorded before and after every treatment. After seven treatments, the collected data were used to evaluate the different between two groups. Volunteers received follow up visits in the third and the sixth months after the seven treatments. Data were used to evaluate the long term effectiveness of two manipulation methods.Result:Two groups were treated by diferent method within two weeks. Treatment group: 36 recovery; 31 improvement; 5 inefficacy. Total effective rate. 93.1%. Control group: 22 recovery; 30 improvement; 18 inefficacy. Total effective rate: 74.3%.Conclusion:The effectiveness of three-dimensional oblique pulling method and the traditional manipulation method is significantly different, P<0.05statistically significant. For the LIDP treatment, three-dimensional oblique pulling method is better than the traditional manipulation method.
