Objective : To evaluate the effect of various nutrition support on the immune function of early postoperative patients with severe acute pancreatitis.Methods: 40 patients with severe acute pancreatitis, APACHE II ≥8 , need operation totally, were divided randomly average into TPN and EN groups. The former received total parenteral nutrition and the latter received enteral nutrition. T lymphocyte subset counts (CD4+ CD8~+ CD56~+ CD4+/CD8+) in peripheral blood were detected by flow cytometry. Intestinal villi were observed by light microscopy and the ultrastructural characteristics of the intestinal epithelial layer of jejunum including tight junction and microvilli, were studied by means of transmission electron microscope.Results: ratio of CD4+/CD8+ in peripheral blood was significantly higher in the EN group than those in the TPN group ( P < 0. 05). Villus height was significantly higher in the EN group than those in the TPN group ( P < 0. 05).Conclusion: enteral nutrition is well tolerated in early postoperative patients with severe acute pancreatitis. It maintains immune responsiveness gut integrity and reduces bacterial and/ or endotoxin translocation bysuppoiting gut integrity.
