Objective:By comparing the influence of early enteral nutrition(EEN) and total parenteral nutrition(TPN) for patients with severe pancreatitis(including in moderate severe pancreatitis and severe pancreatitis) patients in hospitalization, hospital costs, the incidence of infection, serum C-reactive protein(CRP), T cell lymphoma affect Puerto group(CD3, CD4 +, CD4 + / CD8 + value), albumin(ALB), procalcitonin(PCT) and many other indicatorsmethod, explore that under different nutritional conditions support, whether the nutritional status, inflammational status, organ function status, immune function status of the patient are different. We can futher evaluate the clinical significance of EEN in patients with seve acute pancreatitis.Method:Our department in October 2014- January 2016 admitted 40 patients with severe acute pancreatitis were randomly divided into two groups, experimental and control groups. All patients were treated at the time of admission given the same basic treatment, control group received total parenteral nutrition(TPN), the experimental group patients within 72 hours of admission enteral nutritional support. And 1,4,8,12,16 days after admission peripheral venous blood were drawn detect CRP, PCT, renal function, albumin, the percentage of white blood cells and other indicators of the level of neutrophils, 1, 8, 16, measuring T cell subsets. Statistics hospital stay, hospital costs. Application test, repeated measures analysis of variance, two independent samples t-test of experimental data and other statistics. Results:Experimental group and control group of patients during treatment, length of stay and hospital charges no statistical difference. Two groups of patients C-reactive protein, procalcitonin(PCT), the percentage of cells of neutrophils(NE%) decreased during the treatment period, the experimental group and the downward trend is more obvious. Two groups of patients albumin(Alb) level, T cell subsets levels gradually increased, and the upward trend is more obvious in the experimental group. In conclusion:For the severe and acute severe pancreatitis patients given early enteral nutrition support can effectively improve the nutritional status of patients and reduce the degree of systemic inflammation, enhance immunity in patients and reduce the occurrence of late pancreatic infection rates, can effectively improve patients with severe acute pancreatitis prognosis.
