[ Objective ] To investigate the incidence and prognosis-related factors of bleeding stroke in Suzhou and Daxinganling prefectures; to compare the incidence rate and treatment modality between Suzhou and Daxinganling prefectures for this disease; to probe prevention and treatment of choice for this disease.[ Methods ] The subjects of this study were 800 patients with hemorrhagic stroke diagnosed and treated at Suzhou and Daxinganling Prefectures during the period from 2001 to 2003. The age and season at onset, history of hypertension, therapy modality, outcome, as well as cause of death were carefully collected for epidemiological analysis.[Results] (1) Females in both prefectures have lower incidence and higher mean onset age than males. When compared, Daxinganling prefecture witnessed younger (less than 6-7 years) patients but higher incidence than Suzhou, regardless of gender. (2) The incidence was relatively high in seasons when temperature change dramatically in both prefectures. (3) 72.3% of all patients (578/800) were confirmed to have past history of hypertension, the remainder had no blood pressure records before onset or any symptoms related to hypertension. However, it is anticipated that some of the patients suffered from high blood pressure on admission had latent, undiagnosed hypertension. Regretfully, near 70% of patients with hypertension received no any treatment; even the rest patients got remedy, the treatment is neither regular nor systematic. (4) Advanced age, low Glasgow scale, and large hematoma correlated with worseoutcomes, However, it was not advanced age but the resultant reduced physiological status that directly related to bad outcome. (5) When outcomes were analyzed in patients with same Glasgow scale, surgical treatment resulted in remarkably lower mortality rate and better outcomes than medical therapy; moreover, keyhole surgery was superior to other surgical approaches. (6) Surgical intervention at acute stage contributed to lower mortality rate while delay in surgery often leaded to unsatisfactory results.[Conclusion] (1) Hemorrhagic stroke in Daxinganling prefecture is a very serious medical problem when compared with Suzhou prefecture. It is necessary though not sufficient to promote health education, hypertension therapy, as well as diet. (2) Time option and indication for surgical treatment is critical to outcomes. (3) Patients with advanced age should not be curtly precluded from surgery. Better outcomes could be achieved when appropriate surgical methods and timing were chose in the elderly who had a good general condition. (4) Minimally invasive surgery is effective methods for dealing with hemorrhagic stroke.
