Improve The Medical Staff's Cognition On Cancer Pain Management To Affect The Quality Of Life Of Advanced Cancer Patients With Pain

Pain is one of the most important factors that affect the quality of life(QOL) of the cancer patients. It is the key to manage the cancer pain and improve the QOL effectively that both attitude and range of knowledge the medical staff keep it in their minds. The aim of our research was to evaluate the medical staff's level of attitude and knowledge on cancer pain by researching on the medical staff of a tumor rehabilitation of a A-level Grade 3 Hospital with questionnaire and recording the pain status and the QOL of the patients with advanced cancer. After that, we improved the medical staff's level of knowledge on cancer pain management by lectures and clinic training. We analyzed the medical staff's level of attitude and knowledge affecting on the pain status and quality of life of the patients with advanced cancer in order to give a scientific plan about cancer pain management. This research includes three parts.In the first part, we used the paper roll that 138 doctors and nurses of a tumor rehabilitation unit have been investigated. Outcomes: Both the doctors and nurses had active attitude on cancer pain management. There were not significant difference on the level of cancer pain knowledge between doctors and nurses. But they were both lack of knowledge on common knowledge about cancer pain and opioid addiction and drug's side effects and console and so on before our education intervention. We also investigated 95 patients (group A) about their pain status and QOL. Outcomes: the pain level and pain duration were not control effectively. The QOL of group A did not change significantly. It suggested that we should give education intervention to improving the medical staff's level of knowledge en cancer pain management and to improve the QOL of the patients in turn.In the second part, we give a one-week education intervention to the medical staff according the outcome of the research. Four specialists on cancer pain management gave lectures from 2:30 to 5:30 every afternoon. Every morning they guided the medical staff with clinic work. The lecture is main about three-grade analgesia of WHO. The medical staff participated actively and more than 80% of them listened to the lecture every day.The third part is evaluation of the education intervention. We investigated the medical staff and the patients (group B) once more. Outcomes: the knowledge on cancer pain management of the medical staff was improved effectively. The pain status of group B was significantly different from group A. For group B, the pain level was lower and the pain duration was shorter. Five aspects of QOL of group B such as Role Functioning, Emotional Functioning, Global Health, Pain and Insomnia were improved significantly. Improving the medical staff's level of knowledge on cancer pain management could alleviate the pain of patients and improve the QOL in turn.
