The Application And Effect Evaluation Of Knowledge-Attitude-Practice Model On Health Education Of Patients With Enterostomy

Objective:To help patients with enterostomy understand the relevant knowledge of enterostomy in a short time,establish a belief of stoma self-management,to improve patients' self-care ability at home,lower stoma related complications and improve quality of life post operation.Method:In this study,70 patients admitted to the department of gastrointestinal surgery and received enterostomy surgeries in an affiliated hospital of Kunming Medical University from December 2016 to October 2017 were enrolled.According to the sequence of admission time,we randomly divide patients' number into two groups.The odd numbers were divided into the intervention group and even number into the control group.The control group received routine health education.The intervention group was given KAP-based health education intervention program.The variables include self-care agency,incidence of stoma-related complications and Stoma-QOL.Results:The two groups of patients with general information,self-care ability and Stoma-QOL questionnaires before the intervention showed no difference(P>0.05).Self-care agency comparison:The scores of self-care agency of two groups of patients gradually increased after 1 month and 3 months post operation,but the intervention group was significantly higher than the control group,the difference was statistically significant(P<0.05).In the incidence of stoma related complications,the incidence of ostomy skin complications was highest at each time point in both groups,with 37.14%and 60%respectively at 1 month and 17.14%and 40%respectively at 3 months after operation.However,the incidence of overall ostomy complications decreased at each time point,and the effect of the intervention group was significantly better than that of the control group(P<0.05).In terms of quality of life(QOL),there was no difference in both groups before the intervention(P>0.05),but it is definitely different(P<0.05)between the two groups at 1 month and 3 months post operation.Conclusion:KAP Model has been identified as a positive healthcare intervention program that can significantly enhance newly formed stoma patients' overall self-care abilities,reduce the rate of stoma related complications and improve QOL after surgery.
