Objective To explore the incidence, possible mechanism, risk factors, clinical symptoms and clinical significance of leukoarasis in patients who are different age, sex and type with cerebral vascular disease. We analyzed cranial MRI in all kinds of patients and combined their detailed history and correlative laboratory investigations by reviewed.Methods 142 patients with cerebral vascular disease were referred in neurological department of our hospital between January 2003 and October 2004. Patients were divided four groups (A, B, C, D group) according their ages , the two sub-groups (A1 and A2, B1 and B2, C1 and C2, D1 and D2 group) according their sex. At the same time, patients were also divided Cerebral hemerrhage group and Lacunnar infarction group , cortical infarction group and Subcortical infarction group (K, N and L, M group) according the type of stroke. And patients were also divided leukoarasis group and non-leukoarasis group (X and Y group). In addition, 60 patients were divided three groups (â… , â…¡, â…¢ group) according the scores of Mini-Mental Sate Examination (MMSE) between June 2004and October 2004. All patients must be examined brain MRI in one week. Equal or low sign in T1W1 or high sign in T2W1 on MRI around cerebral ventricles and semi-over center were observed , Meanwhile we also acquired detailed knowledge of the history and correlative laboratory investigations.Result (D Of the 142 cases , MRI studies discloses LA were 76 (53. 52%), The incidence was distinct in the different age , and the rate increased following by growing age (P<0.05). The incidence of A, B, C, D group was 11 (28. 95%), 16 (38. 09%) , 28 (70. 00%) and 21(95.45%) .respectively.? There was no difference when different sex were compared with each other in all ages (P>0. 05). (3) Of the 33 cases who were Cerebral hemerrhage , there were 21 occurance cases (63.64%) and of the 44 cases who were Lacunar infartion , there were 35 occurance cases (79. 54%), Of the 24 cases who were Cortical infartion , there were 7 occurance cases (29. 17%) , Of the 41 cases who were Subcortical infartoin ,there were 13 occurance cases(31.71%), there was statistical difference in K, N and L,M group(P<0. 05). ?Congnitive decline was gradually progressed with the severity of LA , there was difference in each other (p<0. 05). (5) Of the leukoarasis group, the incidence of hypertention was 55. 26%, But the non- leukoarasisgroup , the incidence of hypertention was 34.85% .there was difference in each other (P<0.05).Conclusion Long-term hypertention is the most reason of LA , The mechanism of LA is demyelination of cerebral white matter caused by ischemia , by poxia around cerebral ventricles and semi-over center. But diabetes , heart disease , smoking , hyperlipidemia and so on are not ignored .with the growing of age , the incidence of LA increases .LA was correlated with small vascular and LA had significant effect on cognitive decline .Because the emergence of leukoarasis means brain hurt , but there is no special ,way to cure it. Combined the history , LA is helpful to diagnose for patients .
