| Objectives To investigate the cognitive status in the aged with subclinical hypothyroidism and whether these changes are reversible when the euthyroid state has been attained after treatment with thyroid hormore for six months . Methods 50 old preliminarily diagnosed subclinical hypothyroidism newly were randomly assigned to take L-thyroxine or placebo for 6 months according to the sequence . We enrolled 25 euthyroid controls with the same age, gender and education as the patients screened for good general health. The subjects were evaluated for thyroid function and tested with The Cambridge examination for mental disorders of the elderly (CAMCOG-C) and Mini-Mental State Examination (MMSE) before and after treatment . Results Subclinical hypothyroid old patients compared with the same age and education controls showed a significant score reducing on CAMCOG-C full score, attention, remember and perception and similar score on MMSE in baseline (P <0.05 ) . And although thyroxines were in normal ranges, but compares with the normal person all in the low levels before treatment. After six months of thyroid hormone replacement therapy, the scores on CAMCOG-C full score, remember, language and think increase achieved the significance in those patients with TSH>10 mIU/ L; the same situation was occurred except language in those patients with TSH<10 mIU/ L. The increase in all tests in CAMCOG-C and MMSE did't achieved the significance in placebo group(P >0.05)o In order to further eliminate the placebo effect, we compared the enhancement before and after treatment in score in each patients between the L-thyroxine group and the placebo group . As the result , the enhancement in CAMCOG-C full score, remember and think after treatment with L-thyroxine was marked than with placebo in those patients with TSH<10 mIU/ L.
