Objective Toprovide more evidence-based adjustments of HIV/AIDS strategies by furtherunderstanding the epidemic evolution and impacts of responses over years in Fuyang City, Anhui province, which couldalsobeareferencefor other regions with similarsituation as Fuyang in terms of HIV/AIDS control and prevention.MethodsAll data were extracted from the National HIV/AIDS Comprehensive Response Information Management System and analyzed with software SPSS 17.0. Theepidemic evolution between 1995 and 2013were portrayedwith descriptive analysisand trend chi-square test.Demographic characteristics of cases infected by illegal blood trade and those by heterosexual contact were compared with chi-square test to explore whether there was alikely association between these two ways. Some of primary indicators demonstratingthe effectiveness ofHIV/AIDS responses were employed in this study to understand the differences between Fuyang, Anhui and national level.ResultsThis study revealed HIV/AIDS epidemic evolution in Fuyang City from 1995 to 2013. Approximately 66% of cases living with HIV were concentrated in 4 rural counties. More than 47% of cumulatively reported caseshad commercialblood donatinghistory.The standardized crude prevalence of HIV positive was less than1‰. The association between former commercial blood donating transmission routes and heterosexual behaviorswas not observed in this study. After the implement of three-tier follow-up management mechanismforHIV/AIDScases, the indicators of follow up, as well as coverage rate of (anti-retroviral treatment, ART) were significantly improved, and the mortality of having ART cases was decreased.ConclusionsFormer commercialplasma donation used to drive the HIV/AIDS epidemic until China enacted the blood safety law and took action to crack down illegal blood trade in 1998.Thereafter, Fuyang performed HIV screen test as well as other"Four Free One Care"measures among former plasma donors to block the way of HIV transmission through unsafe blood selling. Currently,HIV/AIDS epidemic remained stable in Fuyang city over years and heterosexual contact became the dominant HIV transmission route among newly reported cases. The three-tiers of follow-up mechanism established by local government effectively improved HIV/AIDS case management and guaranteed the quality of ARV treatment which resulted in reduction of fatality and secondary transmission.
