Medical health service accessibility refers to the ability to continue to meet the population can bear in the cost, easy to obtain the form of demand in health service, including the service supplier and demander using two services. The study points out that, as China’s industrialization, urbanization and population aging speed, people have a clear upward trend of medical and health service demand. However, the problem cannot be ignored is the primary health service utilization level is very backward, rural health service can seriously affect the health of g rassroots services to the people of poor and improve. Through the analysis of discussion between g rassroots health service utilization and supply connection, found to hinder the g rass-roots health services can be improved and the influence factors and their mechanism of action, for the integ ration of the regional health resources allocation, the implementation of national basic health policy, improve the primary service to provide basis for health service utilization service g roups, and put forward the countermeasures and suggestions.Firstly, the study is to analyze the current situation and theoretical basis of the accessibility to health services in both domestic and foreign through collecting the documents. This is not only provide theoretical support for scientific evaluating the rural accessibility to health services, but also make up the evaluation index system by using the existing research results. Secondly, analyze the accessibility to health services of rural residents in Yunnan province from 2003-2013 by using the relevant data of China’s Health Statistics Yearbook, China Statistical Yearbook and China Rural Statistical Yearbook. This research mainly analyzed from four aspects, including potential accessibility, realization accessibility, equal accessibility and inequality accessibility to the rural health services. Statistical analysis method includes the descriptive statistical analysis method and factor analysis method.The research found that:(1) Potential accessibility: the geog raphical accessibility to health services is lower in rural residents of Yunnan province. The accessibility to health services of rural residents is lower than that of urban residents, and the gap is mainly encountered in the shortest time period and the shortest distance. Further study found that the factors of the resources allocation of rural health services and the ability to health service of township hospitals and village make a significant effect on promoting the accessibility to health services. Among them, the factor of the resources allocation of rural health services is more important, accounting for 64.5% of the total weight.(2) Realization accessibility: the factors of the utilization of patient, inpatient and preventive health services make a remarkable effect on promoting the accessibility to health services, especially the factors of the utilization of inpatient and preventive health services are more important, accounting for 70.3% of the total weight.(3) Equal and inequality accessibility: the peasants’ economic accessibility, the safeguard ability of the medical security system providing by government, and the choices of medical treatment of rural residents are the main factors of the equal and unequal accessibility to primary health services, but the factors of the peasants’ economic accessibility and the level of medical care are more important, accounting for 79.8% of the total weight.
