Objective:With the development of society and the improvement of people's living standards,residents' requirements for medical and health services are getting higher and higher.Medical staff,as the main provider of medical and health services,especially clinicians,are responsible for saving lives and preventing diseases.In the new medical environment,not only the clinicians are required to have solid professional knowledge,but also have rich clinical experience and superb operating skills,and even understand and sympathize with patients,and the requirements for clinicians are getting higher and higher.However,whether the clinician can reach the patient's high expectations,and whether the patient can understand the basic knowledge of medicine and understand the pressure of the clinician's work,these two aspects are related to the relationship between doctors and patients,which may be the tension between doctors and patients today.The reason is.As the relationship between doctors and patients becomes more and more tense,the pressure on clinicians is getting bigger and bigger.Now clinicians have been recognized as a very stressful group,not only facing a strong workload and the patient's treatment effect.Expectations,but also assume responsibility from the family and society.Clinicians are people's life and health,and their high-tech,high-input characteristics,as well as the particularity and seriousness of service purposes,determine that clinicians are one of the most stressful occupations.The professional group of clinicians is often in a state of high tension in the spirit,which affects the quality of their work and their medical behavior.In addition,there are frequent incidents of medical disputes in the society,but few people care about the inner feelings of the doctors in the medical disputes and the enormous psychological pressure they bear.The psychological condition of doctors is likely to have a certain impact on their job burnout.Studies have shown that the community has high expectations for doctors,the compensation and the pressures they bear,the mismatches,the disharmony between doctors and patients,and insufficient social support.Both may be important predictors of doctors' burnout,but these factors also affect the doctor's mental state,coping style and stress behavior.In clinical work,it is of great significance to explore the psychological status of clinicians and their coping styles.They can guide and help them to choose the correct coping styles by understanding and mastering their psychological status at any time,and reduce inappropriate or harmful ones.Exciting behavior.Medical work is a complex and interpersonal-sensitive job.Clinicians face patients,colleagues,and society as a whole.They are subjected to various pressures every day.The relationship between doctors and patients is highly concerned.Doctors are faced with various situations.Different mental states are manifested,and different coping styles are generated,reflecting different stress behaviors.Because clinicians have many influencing factors on the stress response and coping styles of different states,there are few studies at home and abroad in this direction,and there are fewer studies on the relationship between them.Therefore,this study intends to explore the relationship between the three doctors and their respective influencing factors through the study of the clinician's psychological status,coping style and stress response,in order to find effective means to improve the working status of clinicians,improve work efficiency,and alleviate doctors and Contradictions between patients.Methods:Using cluster sampling methods,14 hospitals in Liaoning Province were selected,including Anshan General Hospital,Anshan Affiliated Hospital,Anshan Shuangshan Hospital,Bengang General Hospital,Benxi Central Hospital,Chaoyang Central Hospital,Dandong First Hospital,Clinics of the Electric Power Center Hospital,Fushun Central Hospital,the General Hospital of the Mining Bureau,the Second Hospital of Liaoyang City,the Liaoyang Central Hospital,the Tieling Central Hospital,and the Fourth Affiliated Hospital of the China Medical University were selected as research subjects.On the premise of informed consent of the respondents,a total of 1,100 questionnaires were distributed,and 1051 valid questionnaires were valid,with an effective rate of 95.55%.In this study,the cross-sectional survey method was used to study the general mental health,trait coping style,and stress response factors of the subjects using the GHQ20,TCSQ,and SRQ scales as tools to explore the relationship between the three.To further construct a predictive model of general mental health and trait coping style response to stress and stress.Results:Reliability and validity of general mental health scale,trait coping style scale and stress response scale.The Cronbach's ? reliability coefficient of the general mental health scale is 0.777,and the Cronbach's ? reliability coefficient of the other three dimensions is in the range of 0.665~0.826.The structural validity of the scale is good,and the factor loading of the three common factors is extracted.The structure of the scale is basically the same,and the success rate of set validity and discriminant validity is 83.33%~100%.The Cronbach's ? reliability coefficient of the trait coping style scale is 0.794,and the Cronbach's ? reliability coefficient of the other two dimensions is 0.880 and 0.853.The structural validity of the scale is good,and the factor loading of the two common factors is extracted.The structure of the scale is basically the same,and the success rate of the set validity and the discriminant validity are both 100%.The Cronbach's ? reliability coefficient of the stress(pressure)response scale is 0.972,and the Cronbach's ? reliability coefficient of the other three dimensions ranges from 0.859 to 0.956.The reliability of the scale is very good;The degree is good,and the factor load of the three common factors extracted is basically the same as that of the scale.The success rate of the set validity is 100%,and the success rate of the discriminant validity is between 83.33% and 100%.The status of clinical doctors' mental health and its influencing factors.The general health status of clinicians: self-affirmation score was 11.79±2.23,depression score was 10.11±1.45,and anxiety score was 8.48±1.77.Factors affecting self-affirmation include family membership,family burden,monthly income,whether you want to leave the unit,colleagues,whether there is a major change in a year,the frequency of physical exercise,whether you have chronic diseases,or different emotional changes.The influencing factors of depression include age,family burden,professional title,length of service,relationship between colleagues,and emotional changes.The influencing factors of anxiety include family membership,family burden,monthly income,whether you want to leave the unit,colleagues,whether there is a major change in a year,the frequency of physical exercise,whether you have chronic disease,or emotional changes.3.The status of clinician coping style and its influencing factors The current status of coping styles of clinicians in this study: positive response score was 32.47±8.68,and negative response score was 24.86±7.93.Factors affecting positive response include marital status,education level,family member relationship,family burden,different departments,different professional titles,ideas about whether to leave the unit,colleagues' relationship,frequency of physical exercise,chronic illness,and emotional changes.Factors affecting negative coping include age,marital status,family member relationship,family burden,job title,length of service,whether you want to leave the unit,colleagues,exercise frequency,and emotional changes.4.The status duo of stress(pressure)reaction of clinicians and its influencing factors.The current stress response of clinicians: emotional response score 16.46 ± 8.44,physical response score 17.17 ± 8.08,behavioral response score 12.32 ± 5.63,total score 49.60 ± 22.44.Factors affecting emotional response include education level,family member relationship,family burden,job title,length of service,monthly income,whether you want to leave the unit,colleagues,frequency of physical exercise,chronic illness,and emotional changes.The factors affecting the body reaction are age,education level,family member relationship,family burden,job title,monthly income,whether you want to leave the unit,colleagues,whether there is a major change in one year,the frequency of physical exercise,whether you have chronic disease Emotional changes.Factors affecting behavioral response scores include family membership,family burden,job title,monthly income,whether you want to leave the unit,colleagues,whether there is a major change in a year,the frequency of physical exercise,whether you have chronic disease,or emotional changes.The factors affecting the total score of stress response are education level,family member relationship,family burden,job title,length of service,monthly income,whether there is any idea of leaving the unit,colleague relationship,whether there is a major change within one year,the frequency of physical exercise,whether or not There are chronic diseases and emotional changes.5.Relationship between clinician's mental health,coping style,stress response.The three dimensions of stress response were significantly correlated with general mental health and trait coping styles(P<0.05).The stress response score,emotional response score,physical response score,and behavioral response score were positively correlated with self-affirmation scores in general mental health,negatively correlated with depression scores and anxiety scores,and positive with trait coping styles.The response scores were negatively correlated and the negative response scores were positively correlated.The two dimensions of trait coping style and general mental health also showed a significant correlation(P<0.05).Positive response scores and negative coping scores were negatively correlated with self-affirmation scores in general mental health,with depression scores and anxiety.The score is positively correlated.6.The predictive effect of general mental health and trait coping styles on stress response.The three dimensions of general mental health(self-determination,depression,anxiety)scores,two dimensions of trait coping styles(positive coping,negative coping)scores have predictive effects on the physical response of stress response;general mental health The two dimensions(self-affirmation,anxiety)scores and the negative response scores in the trait coping styles have predictive effects on the emotional response to stress responses;three dimensions in general mental health(self-affirmation,depression,anxiety)The negative scores in the scores and trait coping styles have predictive effects on the behavioral responses of stress responses;the three dimensions of general mental health(self-affirmation,depression,anxiety)scores,and the negative responses in trait coping styles The score has a predictive effect on the total stress response.Positive responses and negative responses play a mediating role between mental health and stress response.Conclusion:1.The general mental health of clinicians is good.The factors affecting their mental health mainly include age,family burden,monthly income,relationship with family members,whether they want to leave the unit,their relationship with colleagues,the frequency of physical exercise,And emotional changes.2.Among the clinician's trait coping styles,the number of active responders is higher than the number of passive responders.The factors affecting the coping style mainly include age,marital status,education level,relationship with family members,family burden,whether they want to leave the unit,colleagues' relationship,frequency of physical exercise,and emotional changes.3.The clinician's stress(pressure)response level is good.Factors affecting the level of stress are mainly family members,family burden,monthly income,whether you want to leave the unit,the relationship with colleagues,the frequency of physical exercise,whether you have chronic diseases and emotional changes.4.The stress response and the general mental health and trait coping styles all showed obvious correlations,and the trait coping styles and general mental health also showed obvious correlation.General mental health and trait coping styles have a certain predictive effect on stress(pressure)response.Positive responses and negative responses play a mediating role between mental health and stress response.
