Core Competence In Medical Support Of Army Division Hospitals Based On The Theory Of Preserved Military Health Service

As the traditional military concept and the security ideas have been impacted by the new reform of military,tremendous changes will happen in the future war.Given the increasing military fields applications of information technology,biotechnology,and nonmaterials technology,new frontier,high frontier and biological microsystems frontier will become new sovereignty concepts as well as important guarantees of national security.With the risk accumulation of China's surrounding security is increasing,the CPC Central Committee has made significant strategic decisions to comprehensively build the national defense and army affairs in the way that set up the army leading institutions to speed up the army transformation.For changes of warfare forms and increases of the army tasks,a brand-new combat effectiveness model was formed.That is how the medical service corps need to walk out to the frontline,guaranteeing corps gain scientific,systematic and effective medical service with higher standards and greater challenges.With the changing of ideas of the “low casualties” and even “zero casualty” on the affordability of casualty on the construction of medical service,the goal of combat aid technology is focused more on making the holistic wellness than saving lives.Besides,as the injury means and striking distance,damage degree and other factors are able to be controlled in the future high-tech warfare,the missions of medical service will put more emphasis on the elevation of combat effectiveness,guarantee mechanism and pre-protection than casualties curing.The key scientific problem of this research is to maximize the effectiveness of preserved military health service(PMHS)in the new military revolution.Because it will be possible to gain the breakthrough development and effective utilization of combat aid technology by deepening the theory thinking of PMHS,instructing the building of actual combat medical service under the background of the brand-new combat theory and the boom of medical technology.In addition,as the core strength of our army's medical service,army division hospitals are not only the main stage for war rescue,instruction,training,medical and military operations other than war(MOOTW),but the key point which covers nearly all medical services in the hierarchical diagnosis and treatment system.Thus,it is an ideal place to practice and promote the theory of PMHS.Based on the theory of PMHS,this research aims to explore the construction of core competence of medical support in army division hospitals,focusing on our military historical missions in the new century and new stage.It will be a benefit to provide scientific decision support for military operations of medical service under the condition of real combats.The related theories and subject study methods of management and military science,operations research and statistics were employed to elaborate the concepts,core connotation and application prospect of PMHS theory.Combining the suggestions of experts and analysis of mathematical modeling,the author researches components and interrelationships of PMHS to construct the system of core competence of medical support of army division hospitals.Afterwards,to establish the capacity system,the Real Combat Medical Service Simulation Training Center(RCMSST)was built with reference to the Modeling of Combat Resource Scheduling and Assistant Decision-Making Methods and the virtual simulation technology.By participating in joint operations training and MOOTW of medical service practices,the author carries out the empirical research on practice effect and gives the considerations and prospects of the construction of core competence in medical support(CCMS)of army division hospitals.In the part of status research and theoretical analysis,the paper analyses the basic situations,related theories and development advantages of foreign force and our force based on an extensive investigation of domestic and international literature.Taking the missions of real combat medical service as the research objects,the author summarizes the construction experience and existing problems of core competence of medical service in military hospitals by the methods of case study and analogy analysis.Furthermore,under the conditions of modern war in this new stage,the time points,injury characteristics and treatment points of medical support process of army division hospital are explored.Ultimately,this part makes an explanation of the important concepts like PMHS and CCMS,compares the PMHS with theories of foreign military medical service theories and studies the new characteristics and trends of the theory application in the new point.In the part of system research on the core competence of medical support,the methods of information analysis and typical case,panel discussions and brainstorming were adopted;Through above methods,the author deeply understands the development strategy of the army transformation,the expansion of tasks and the construction of new quality combat effectiveness.On basis of the above analysis,the two rounds of questionnaires were designed by the Delphi method,which 60 experts who work in fields of management and medicine,military and engineering of the whole army related units were enrolled.The questions such as the components of theories of PMHS,CCMS for army division hospitals were contained.By the methods of analytic hierarchy process,exploratory factor analysis and entropy,the researcher sets the index and weight system of CCMS based on PMHS with the collected data.The results are as follows: efficiency of medical service,initiative of medical service and technology of medical service are three interrelated core measures to advance the reform of PMHS.In the part of research on the construction practice of the core competence of medical support,based on the construction of efficiency of medical service,initiative of medical service and technology of medical service in PMHS,the author builds the real combat medical service simulation training center(RCMSSTC)by the scientific allocation from resource to tasks applying the military model of multi-priority combat resource scheduling and assistant decision-making method and the application of the blues of medical service.The resource collection of virtual simulation synthetic training of on-site aid,transform to rear,comprehensive aid,field surgery,intensive care unit(ICU)and controlled assessment are formed;By applying the technologies of medical simulation and computer simulation all around,the medical combat simulation training was carried out on the situation of unpredictable injury and competitiveness;Meanwhile,the author figures out the real combat modes of combat aid ability like physiological and psychological adaptation,cooperation awareness,and complex treatment abilities,basic ability of combat aid and thinking of combat aid;Through network learning,model operation,virtual simulation scene exercises,the mobile medical service unit went on the competitive training on model of multi-task simulation scenes;Finally,the unit,combined and modular training mode was innovated.While enhancing our corpsmen's practical ability to analyze and solve problems under the complex and multitask situations,the author has a research on capacity system model of PMHS,which is suitable for generating the new quality combat forces.In the part of the empirical research,based on the military model of multi-priority combat resource scheduling and assistant decision-making method and the resource collection of the RCMSSTC,China-foreign joint military medicine drills,the plateau joint combat drills,medical rescue of supporting Africa in fighting Ebola were systematically accomplished by making and training the best schedule of medical service.Through the empirical research methods,the model of building CCMS based on PMHS is improving.Finally,the author puts forward three suggestions from the perspective of the army tasks extension and initiative of strategies,multiple military tasks in the future,and especially the PMHS capacity system construction for generating new combat effectiveness: first,the top design and system integration should be strengthened to realize the sustainability of medical service effectiveness;Second,the mechanism of initiative medical support should be built to make professional decisions on the real combat;Third,the scientific and technological innovation platform and system should be reconstructed to promote the applications of advanced science and technology on the military medical service and create good conditions for quick transformation from the construction of CCMS in the army division hospital to PMHS one in the new era.
