| Constipation is an intestinal disease characterized by difficulty in defecation and dry stool.Acne is a skin disease characterized by facial skin lesions.Epidemiological studies in recent years have shown that the incidence of constipation combined with acne is increasing year by year,which has attracted widespread attention.However,there are few related studies at present.Whether constipation is related to acne or not,and whether there is a connection hub between them.How effective Chinese medicine is in treating constipation with acne is an urgent problem to be solved.On the one hand,this article observes the clinical efficacy of self Liqi Tongbian Recipe intervention in the treatment of Qi stagnation syndrome constipation combined with acne through clinical research;on the other hand,through the establishment of animal models of constipation combined with acne model,and then intervention of Qi stagnation syndrome constipation combined with acne animal model by self Liqi Tongbian Recipe,to explore the correlation between constipation and acne and the connection mechanisms between them,and to analyze the possible mechanism of Liqi Tongbian Recipe in treating Qi stagnation syndrome constipation combined with acne.Clinical StudyClinical study on self Liqi Tongbian Recipe intervention for Qi stagnation syndrome constipation combined with acneObjective:To observe the clinical efficacy of self Liqi Tongbian Recipe in the treatment of Qi stagnation syndrome constipation combined with acne.Methods:Sixty patients with constipation of Qi stagnation syndrome constipation combined withe acne in outpatient department of Dong Fang Hospital were randomly divided into experimental group and control group with 30 cases in each group.The experimental group took self Liqi Tongbian recipe orally and the control group took mosapride tablets orally.The constipation score,acne score and TCM symptom score of the two groups before and after treatment were counted.The difference of curative effect of constipation and acne between the two groups and the correlation between constipation and acne curative effect were analyzed.Results:There were 29 cases in the experimental group and the control group.There was no adverse reaction in the experimental group and 1 adverse reaction in the control group.After treatment,the total constipation score,total acne score and TCM symptom score were lower than before treatment,there was statistical significance(p<0.05);the total effective rate of constipation in the experimental group was 82.76%,and the total effective rate of constipation in the control group was 72.41%,there was no significant difference between the two groups(p>0.05).The total effective rate of acne in the experimental group was 79.31%.The total effective rate of acne in the control group was 58.62%,there was significant difference between the two groups(p<0.05).The total effective rate of acne in the constipation improvement group was higher than that in the constipation no improvement patients,the difference was statistically significant(p<0.05);there was a positive correlation between constipation and clinical efficacy of acne(r=0.57,p<0.05).Conclusion:The clinical efficacy of Liqi Tongbian Recipe in the treatment of Qi stagnation syndrome constipation combined with acne is certain,constipation and acne have a correlation in clinical efficacy.Experimental Study1.Experimental study on the relationship between constipation and acneObjective:To establish an animal model of constipation combined with acne,and to explore the correlation between constipation and acne and its connection hub.Methods:Forty rats were divided into normal group,constipation group,acne group and constipation combined with acne group(composite group).Constipation combined with acne model was established.The general condition of the animals,number and weight of feces in 24hours,ear skin lesions and tissue HE staining were observed.The TLR2 and TLR4 in rat auricle tissues was detected by immunohistochemistry.The levels of IL-1?,IL-2,IL-4,IL-6,IL-10 and TNF-? in serum were detected by ELISA,and the fecal intestinal flora was detected by 16s diversity.Results:The general conditions of the three groups of animals were significantly different from those of the normal group.The composite group was different from the constipation group and the acne group.The number and weight of feces in the combined group and constipation group were statistically different from those in the normal group(p<0.05);the number and weight of feces in the acne group were not different from those in the normal group(p>0.05).The auricle of the rats in the composite group and the acne group showed skin lesions and histopathological changes,and the auricles of the constipation group was not changes.The levels of IL-la,IL-2,IL-4,IL-6,TNFa in the constipation group,acne group and composite group were higher than the normal group,and the IL-10 content was lower than the normal group,those differences were statistically significant(p<0.05).The levels of IL-1?,IL-2,IL-4,IL-6 and TNF-a in the composite group were higher than those in the acne group and constipation group,and the IL-10 content was lower than the acne group and constipation group,those differences were statistically significant(p<0.05).The average optical density of TLR2 and TLR4 in the auricle of the composite group and the acne group were higher than those in the normal group(p<0.05);The average optical density of TLR2 and TLR4 in the auricle of the constipation group was not different from that in the normal group(p>0.05);The average optical density of TLR2 and TLR4 in the auricle of the composite group was higher than that of the acne group(p<0.05);the richness and diversity of the intestinal flora in the constipation group and the composite group were lower than that in the normal group.The richness and diversity of the intestinal flora in the acne group were not different from those in the normal group.The richness and diversity of the intestinal flora in the composite group were lower than those in the acne group.The richness and diversity of intestinal flora in the composite group were not different from those in the constipation group.The abundance of intestinal flora in the rats in each group was significantly different at the level of Phylum and Genus.The average optical densities of TLR2 and TLR4 were positively correlated with the contents of IL-1?,IL-4,IL-2,IL-6 and TNF-?,but negatively correlated with the contents of IL-10;There was a correlation between the abundance of Proteobacteria?Bacteroidetes?Actinobacteria?Verrucomicrobia?Tenericutes in intestinal flora and the level of inflammatory cytokines.Conclusion:The animal model of constipation combined with acne has been successfully established.Inflammatory reaction and intestinal flora may be be one of the possible mechanisms associated with constipation and acne.2.Experimental study on self Liqi Tongbian Recipe intervention for Qi stagnation syndrome constipation combined with acneObjective:To study the therapeutic effect of traditional Chinese medicine on Qi stagnation syndrome constipation combined with acne rat model,and to explore the mechanism of traditional Chinese medicine in improving constipation and acne.Methods:48 rats were divided into normal group(8rats)and model group(40 rats).After successful modeling,the model group was divided into Blank group,low group,middle group,and high group,Control group.Each treatment group of traditional Chinese medicine was given three doses of low,medium and high doses of traditional Chinese medicine.The control group was given mosapride tablets,and the blank group was given distilled water.The general condition of the animals after treatment,the number and weight of feces in 24hours,the ear skin lesions and tissue HE staining were observed.The content of TLR2 and TLR4in the auricle tissue of rats was detected by immunohistochemistry.The serum IL-1?,IL-2,IL-4,IL-6,IL-10 and TNF-? levels was detected by ELISA,16s diversity to detect fecal intestinal flora.Results:The general condition,number and weight of feces in 24hours,ear lesions and HE staining,inflammatory cytokine levels,average optical density of TLR2 and TLR4,richness and diversity of intestinal flora community,community structure and species abundance at phylum and Genus level of blank group were significantly different from those of the normal group.The general conditions of the Chinese medicine treatment group and the control group were significantly different from the blank group;The amount of feces in 24h of Chinese medicine treatment group and control group were higher than that of the blank group,and the fecal weight was lower than that of the blank group,there differences were statistically significant(p<0.05).The auricular skin lesions,HE staining of the Chinese medicine treatment group and control group were all significantly difference from blank group.The contents of IL-la,IL-2,IL-4,IL-6 and TNF-a in the serum of the Chinese medicine treatment group and control group were lower than those in the blank group,and the IL-10 content was higher than the blank control,there differences were statistically significant(p<0.05).The average optical density of TLR2 and TLR4 in the Chinese medicine treatment group and control group were lower than those in the blank group(p<0.05).The richness and diversity of intestinal flora in the Chinese medicine treatment group and control group were higher than those in the blank group;The community structure of intestinal flora in the Chinese medicine treatment group and control group were significantly difference from blank group;The abundance of some species in the intestinal flora of the Chinese medicine treatment group and control group were statistically different from that of the blank control group(p<0.05).Conclusion:Chinese medicine has a significant effect on the treatment of Qi stagnation syndrome constipation combined with acne rat model.It can significantly improve the indicators of constipation and acne.Regulating inflammatory cytokines and intestinal microbes may be one of the mechanisms of improving constipation combined with acne. |
