The Study Of Usage Of Golden Cypress Included In "the Viewpoint Of A Guide To Clinical Practice"

ObjectiveThe purpose of this research is to summarize the effect of golden cypress application principle comprehensively and to provide definitive basis for modern Chinese clinical correctly apply golden cypress,by studying the application principle of golden cypress in A Guide to Clinical Practice,studying the past dynasties in materia medica literature, Treatise on febrile diseases and Synopsis of the Golden Chamber,analyzing the similarities and differences to a certain extent,and studying how WenBingXue party inherit and carry forward the theory of shang han.Methods We searched the relevant medical cases of golden cypress and prescriptions contain goldencypress in A Guide to Clinical Practice,including courses of diseases, the related diseases, pathogenesis, symptoms, patterns of syndrome, the times of of clinical visits, tongue and pulse conditions, formulated prescriptions and the its usage in each part and chapter,and the compatibility with other drugs. An then we establish database by the application of Microsoft excel,organized the related data and analyzed statistically by the application of IBM SPSS Statistics 20.ResultsWe found that in the relevant medical cases of golden cypress recorded in A Guide to Clinical Practice as following:(1) Golden cypress is used in each volume among all the 10 volumes,but the times used in each volume is difference.It is used the most frequently in volume 7(26 times,25.2% of the total),and then volume 3(21 times,20.4%).(2) Golden cypress is used in 39 chapters among all the 62 chapters.the most frequently used in the chapter named stranguria with turbid urine,followed by impotence,spermatorrhea,diarrhea,constipation,hernia,worms,hematochezia,arthralgia.(3) Golden cypress is used in the whole treatment course in the vast majority medical records(with 80 medical records,77.7% of all 103 records).(4) Times of seeking doctors of medical case treated with golden cypress recorded generally for 1 time, accounted for 66.0% among all the cases,followed by 2 times(15.5%).(5) Golden cypress is used in all the 8 systems of the human body,the most frequently used in digestive system(34 times,33.0%),followed by exercise system,urinary system,reproductive system,nervous system,and circulatory system.(6) The tongue situation is talked about for only 3 times,each of white tongue,dry tongue with white fur,gray tongue with white edge recorded in 1 time.(7) There are 23 parts recorded about pulse situation,which can divided into rapid pulse, combine pulse, deep pulse, feeble pulse, forceful pulse, bounding pulse in sequential decline.(8) The most common pattern of syndrome is sthenia syndrome(caused by damp and hot,toxic heat,excess fire in heart,liver and intestines system),nearly 45%,deficient syndrome(caused by deficiency of yin only or both in liver and kidney system) slightly more than syndrome of intermingled deficiency and excess(caused by kidney deficiency damp-heat,or imbalance between heart-yang and kidney-yin).(9) According to the eight methods of yixuexinwu,moistening tonification is the most frequently used,followed by heat-clearing method,warming and invigorating, etc.(10) There are 103 prescriptions which contain golden cypress,including 4 types fo dosage forms,with decoction 69(67%),pill 31(30%).(11) There are only 9 set prescriptions,among which Pill for Replenishing the Kidney-yin is often seen for 12 times(31.6%),followed by Anwei Pellets 7 times(18.4%),powder 1 time(2.7%).(12) The Chinese herbs combined with golden cypress from multiple times to less in turn are coptis root(28 times,4.1%), tuckahoe, rhizoma anemarrhenae, rhizoma alismatis, radix rehmanniae preparata, radix paeoniae alba, rhizoma chuan, ginseng, cinnamon, tortoise plastron(along with shells), wide leather, achyranthes, gardenia, pericarpium citri reticulatae viride, atractylodes, toosendan fruit, Chinese angelica, etc.(13) There are 7 types of golden cypress recorded,including golden cypress fried black,golden cypress fried,golden cypress fried burnt,golden cypress,golden cypress fried slightly,raw golden cypress and golden cypress fried with salt water.The most frequently used type is golden cypress(93 times,90.3% of the total).(14) There are 8 records about the dosage of golden cypress,from minimum 5 fen to maximum 8 liang(1 liang=37.5g,1 liang=100 fen).Conclusions(1) The clinical cases used golden cypress by Ye Tianshi often got good curative efficacy only after one or two doses of Chinese medicine,and used though out the whole treatment course or in the earlier stage.The remarkable therapeutic effects reflect that Ye Tianshi’s technologies on diagnosis and treatment are very good or reveal that it is not so difficult to use golden cypress well.(2) Though a wide range of disorders were treated by Ye Tianshi. The main effects of golden cypress are consolidating the Yin by heat-clearing and damp-dry, purging fire and removing steam, detoxification and treating sores, bitter and cold,. However, different type of proceeded golden cypress can disperse the effect throught sanjiao, decending the cold of cypress to suit different therapeutic.(3) The main effect of gold cypress are consolidating the yin by bitter and cold, heat-clearing and damp-drying. The indications for Ye Tianshi to prescribe golden cypress are symptoms found by observation such as damp-heat accumulation (bombe,expansion,swelling), dampness-heat fuming or steaming(pale color,for God,fan,go to fall),fire due to yin deficiency(thin),xuhuo phlogistic(red).(4) Ye Tianshi may points out to posterity that it is not contraindication when the tongue situation shows white,dry or gray. It proved that the efficacy of special treated golden cypress changed to suite different therapeutic.(5) Pulse conditions such as slow pulse and irregularly pulse can’t be found in all medical cases used golden cypress,for which we consider that golden cypress should not be used when yang is deeply in deficency.(6) The deficiency of yin and yang, spleen and stomach and cold syndrome may be contraindication to prescribe golden cypress.Ye Tianshi didn’t use golden cypress to purge the pathogenic fire for removing toxin,but to nourish yin to reduce pathogenic fire,not only for chronic syndrome and deficient syndrome,but for severe case and heat-toxin syndrome.(7) The reason why the method nourishing Yin to removing heat is most widely used is because golden cypress is not used as monarch drug in a prescription among these clinical cases.(8) Ye Tianshi is good at combining golden cypress with other Chinese medicine to get the mutual promotion between two drugs or to constraint the febrile dryness of other Chinese medicine.(9) Ye Tianshi prescribed cryptorrhea and tincture contained golden cypress.This innovation of dosage form is worth our later generation learning and to research.(10) Ye Tianshi had inherited and developed the usage of Pill for Replenishing the Kidney-yin created by Lee Dong Yuan.He is bold to make innovative use of it to treat many kinds of diseases such as liver wind,spermatorrhea,drench turbidity,constipation,hernia and the pain diseases,with good therapeutic effects.Ye Anwei pills came from Wumei pills in Treatise on Cold Damage Diseases written by Zhang Zhongjing,which embodied Ye’s inheritance and innovation of classical prescription.(11) Ye Tianshi mainly used pills when he prescribed set prescription contained golden cypress,which reflected his golden cypress medication characteristics such as little dosage usage for each time,and didn’t extract pharmacological components with water but direct swallowed.(12) Ye Tianshi considers that the difference of golden cypress stir-frying degree and accessories can lead to differential change of property and pharmacology of golden cypress.(13) Ye Tianshi’s clinical experience in little dosage usage of golden cypress with good curative efficacy deserves to be further studied.
