Study On Syndrome Differentiation And Treatment Rules Of "Collateral Disease" In A Guide To Clinical Practice With Medical Record

Objective:Conclude and analyze "collateral disease" theory and its syndrome differentiation and treatment rules through screening and sorting medical records related to "collateral disease" in A Guide to Clinical Practice with Medical Record in order to provide a literature reference and new ideas for theory study and clinical practice of "collateral disease".Methods:Based on the method of literature research,searching,arranging and analyzing ancient literature about "collateral disease " theory;and then screening medical records related to"collateral disease",establishing data table of cases and prescriptions with items of disease,treatment,herbs and prescriptions to conclude Ye Tian-shi academic characteristics of"collateral disease" theory and its syndrome differentiation and treatment rules.Results:1.The study involves 258 cases of "collateral disease",including 209 cases belong to internal medicine,taking up 81.01%of the total number;11 cases belong to surgery,taking up 4.26%of the total number;29 cases belong to gynecology,taking up 11.24%of the total number;3 cases belong to pediatrics,taking up 1.16%of the total number;6 cases belong to ENT,taking up 2.33%of the total number.The 209 internal medicine "collateral disease"involve 22 cases of Exogenous diseases,accounting for 10.53%of its total;1 case of Lung disease,accounting for 0.48%of its total;82 cases of Liver and gallbladder diseases,accounting for 39.23%of its total;24 cases of Spleen and Stomach diseases,accounting for 11.48%its total;5 cases of Kidney diseases,accounting for 2.39%of its total;10 cases of Heart disease,accounting for 4.78%of its total;29 cases of Qi-blood-fluid system disease,accounting for 13.88%of its total;36 cases of Body meridian system disease,accounting for 17.22%of its total;There are 55 diseases and syndromes related to "collateral disease";2.The syndrome differentiation method includes 27 cases by eight principles;60 cases by etiology;63 cases by Qi-Blood-Fluid;56 cases by Zang-fu organs;5 cases by Wei-Defense,Qi,Ying-Nutrient and Blood;5 cases by triple energizer;11 cases by meridian;3.Summarize the main etiology and pathogenesis of"collateral disease";4.Representative "collateral disease" including stroke,pain in lateral chest and stomach,Bi Zheng and eight extra channel collateral disease;5.Summarize common Tongluo prescriptions and frequent Tongluo herbs used above 10 times.Conclusions:1.Human body is unobstructed,when "collateral disease " happens,it turns to stagnation,clogging and stasis,which is often accompanied by structure and function injury of collaterals.And then it makes human body atrophic and weak.It has long course,meanwhile,it is hard to cure and easy to relapse."Collateral disease"is widely found in a variety of internal miscellaneous disease.Its material base are Qi,blood and body fluid;2."Collateral disease " is mainly caused by exogenous pathogens,internal injuries caused by seven motions,pathologic products and chronic disease as well as long pain.Other factors include wrong diet,physical overstrain and therapist fault.Its pathologic changes contains Qi or Blood stasis,Chuji or micro concretion in collaterals as well as injury and faulty nutrition of collaterals;3.There are 258 cases of "collateral disease" in Guide to Clinical Medical Records including 55 diseases and syndromes with syndrome differentiation method of eight principles,etiology,Qi-Blood-Fluid,Zang-fu organs and Wei-Defense,Qi,Ying-Nutrient and Blood and triple energizer;4.Ye Tian-shi is good at treating "collateral disease" according to the characteristics of primary disease.Activating channels is the main treatment of collateral disease focusing on course,deficiency and excess.Caring is important for "collateral disease";5.The main Tongluo prescription includes blood activating recipe such as Biejiajian Pill,Xuanfuhua Decoction;tonifying deficiency recipe such as Pulse-Restorative Decoction,Banlong Pill;aromatic recipe such as Bezoar Pill,Zhibao Pill and Zixue Pill.There are 63 kinds of activating channel medicines used over 10 times,in which pungent herbs are most commonly used,which can also be aromatic,lubricate or has the taste of bitter and cold or warm.Herbs have the function of antidote,reducing phlegm,eliminating stagnation and calming liver wind can be used to dredge collaterals.Ye Tian-shi is good at activating healthy Qi and dredging collateral according to the Qi,blood,Yin,Yang condition in collaterals to treat"collateral disease" with deficiency syndrome.
