Objective:Type 2 diabetes mellitus has become a social problem,because of its characteristic of great harming,difficulty to cure, the high cost,it has become a big threaten to the health of human being and now it is the fourth reasons of death.The islet beta cell dysfunction is central to the pathogenesis of type 2 diabetes mellitus. How to protect the islet beta cell function is the key to the treatment of type 2 diabetes mellitus,which is of great significance that the prevention of diabetes and its complications.In this paper,taking advantage of high fat and high glucose diet and the low dose of streptozotocin to replicate animal model of type 2 diabetes mellitus.studing the intervention effect of point injection and point embedding therapy on blood glucose,body weight, serum insulin,insulin sensitivity index,promoting apoptosis pathway of PANDER in type 2 diabetes rats.To explore the mechanism of protecting the islet beta cell function by the point injection and point embedding therapy. Studing mechanism of treatment of type 2 diabetes mellitus by point injection and point embedding therapy from the molecular level.It provide a theoretical basis for the treatment of type 2 diabetes mellitus by point injection and point embedding therapy and clinical screening methods of treatment.Methods:To feed 50 SD male rats for 1 week.the 50 SD male rats were divided randomly into normal group and model group.Normal group was fed with normal diet,model group was fed with high fat and high sugar diet.After 4 weeks of feeding,according to the weight of 35 mg / kg,the male SD rats in the model group was injected streptozotocin on the right side of the abdominal cavity to replicate the model of type 2 diabetic rats.While modeling,normal diet group was injected with citrate buffer solution,after 72 hours each group rats were measured fasting blood glucose,the value of blood glucose more than or equal to 11.1mmol / L was diagnosised type 2 diabetic rats,then the rats that were established type 2 diabetes mellitus model successfully were randomly divided into model group,point injection group,point embedding therapy group. At the same time,the normal diet group rats were used as normal control group.point embedding therapy group was treated with bilateral Zusanli(ST36), Sanyinjiao(SP6),Weiwanxiashu(EX-B3), pointembedding therapy once a week,continuous treatment for 4 weeks,point injection group was injected Shenmai injection in the same point once every other day,continuous treatment for 14 times,a total of 4 weeks.The the model group rats and normal control group rats were with no intervention,but the two groups rats were captured and fixed at the same time with the same method. After 4 weeks treatment,observing blood glucose,serum insulin,insulin sensitivity index of the each groups rats.Pancreatic sections of type 2 diabetes rats were stained with HE,the morphological changes of pancreatic islets was observed. pancreatic tissue of each groups rats were detected the expression of PANDERm RNA,p21 m RNA,Caspase-3m RNA by real time PCR,the expression of PANDER protein,p21 protein,Caspase-3 protein were detected by western blot.The results was comprehensive statistical analysised.Results:The basic biochemical informations of the rats in each group :(1) The model group rats were fed with high fat and high glucose diet for 4 weeks.compared with blank control group,the body weight of the model group rats was significantly increased(P<0.01),the model group rats were injected streptozotocin after 72 weight,compared with the blank control group,the body weight of the model group rats was decreased obviously(P<0.05).(2) From the fasting blood glucose level of each groups,before the treatment of point injection and point embedding therapy,fasting blood glucose level of point embedding therapy groups,fasting blood glucose level of point injection group and fasting blood glucose level of the model group, was sharply higher than the fasting blood glucose level of normal control group,but there was not any significant difference in blood glucose level between the 3 groups.After the treatment of point embedding therapy and point injection,the blood glucose level of point embedding group and the blood glucose level of point injection group were significantly lower than the blood glucose level of model group(P<0.01),the blood glucose level of 2 groups had significant difference.it showed that point embedding therapy and point injection had the function of reducing blood glucose level.blood glucose level of point injection group was lower than the blood glucose level of point embedding therapy group,2 groups also showed a significant difference(P<0.01),which illustrated that the effect of reducing blood glucose blood level of in the point injection group was better than the effect of reducing blood glucose blood level of point embedding therapy group.(3) from the serum insulin level of each groups,the level of serum insulin of the rats in the model group was significantly higher than the level of serum insulin in the normal control group(P<0.01),the level of serum insulin of the rats in the point injection group and the level of serum insulin of the rats in the point bedding therapy group were obviously lower than the level of serum insulin of the rats in the model group(P<0.01),the levels of serum insulin of the rats in point injection group was lower than the level of serum insulin of the rats in the point embedding group(P<0.01).These results showed that point injection and point embedding therapy could reduce the serum insulin level in type 2 diabetic rats effectively.The point injection on reducing the action of the type 2 diabetes rats serum insulin was better than the point embedding therapy on reducing the action of the type 2 diabetes rats serum insulin.(4) from the insulin sensitivity index level of each groups,the level of insulin sensitivity index in model group rats was significantly lower than the insulin sensitivity index level in normal control group(P<0.01),which showed that the model group had insulin resistance,the the level of insulin sensitivity index in point embedding and the the level of insulin sensitivity index in therapy group rats were higher than the the level of insulin sensitivity index in the model group(P<0.01),which showed that the point injection and acupoint embedding therapy may improve insulin resistance in type 2 diabetes mellitus rats.insulin sensitivity index in point injection group rats were higher than the the level of insulin sensitivity index of embedding group rats(P<0.01),there was statistical significance.These results showed that point injection on the improvement the effect of insulin resistance of type 2 diabetic rats is better than the improvement the effect of point embedding therapy.2. In the same vision of optical microscope,islet cell damage in the pancreatic tissue of point embedding therapy group rats and islet cell damage in the pancreatic tissue of point injection group rats was less than that in the model group,islet cell damage in the pancreatic tissue of point injection group was less than that in point embedding group.3. Effects of rats in each groups of the expression of PANDERm RNA in pancreatic tissues: compared with the normal group,model group increased expression of PANDERm RNA in the pancreas,the difference was remarkably (P<0.01);compared with the model group rats,pancreatic PANDER expression of m RNA in point embedding therapy group and point injection group were decreased.The difference was significant(P<0.01);compared with the embedding therapy group,the expression of PANDERm RNA expression in pancreas tissues of the point injection group was decreased,the difference was significant(P<0.01).4. Effects of rats in each groups of the expression of PANDERm RNA in pancreatic tissues :compared with the normal group,model group increased expression of PANDER protein in the pancreas,the difference was remarkably(P<0.01);compared with the model group rats,pancreatic PANDER expression of PANDER protein in point embedding therapy group and point injection group were decreased.the difference was significant(P<0.01); compared with the embedding therapy group,the expression of PANDER protein in pancreas tissues of the point injection group was decreased,the difference was significant(P<0.01).5. Effects of rats in each groups of the expression of p21 m RNA in pancreatic tissues :compared with the normal group,model group expression of P21 m RNA in the pancreas was decreased,the difference was remarkably(P<0.01); compared with the model group rats,p21 expression of m RNA in point embedding therapy group and expression of p21 m RNA in the pancreas of point injection group were increased.the difference was significant(P<0.01); compared with the embedding therapy group, the expression P21 m RNA expression in pancreas tissues of the point injection group was increased,the difference was significant(P<0.01).6. Effects of rats in each groups of the expression of p21 protein in pancreatic tissues :compared with the normal group,model group expression of p21 protein in the pancreas was decreased,the difference was remarkably(P<0.01); compared with the model grourp rats,p21 expression of protein in point embedding therapy group and expression of p21 protein in the pancreas of point injection group were increased.the difference was significant(P<0.01); compared with the embedding therapy group,the expression p21 protein expression in pancreas tissues of the point injection group was increased,the difference was significant(P<0.01).7. Effects of rats in each groups of the expression of Caspase-3 in pancreatic tissues :compared with the normal group,expression of Caspase-3 m RNA in the pancreas of model group were augmented,the difference was obvious(P<0.01);compared with the model group rats,Caspase-3 expression of m RNA in point embedding therapy group and Caspase-3 expression of point injection group were decreased.the difference was significant(P<0.01); compared with the embedding therapy group,the expression of Caspase-3 expression in pancreas tissues of the point injection group was decreased,the difference was significant(P<0.01).8. Effects of rats in each groups of the expression of Caspase-3 protein in pancreatic tissues:compared with the normal group,expression of Caspase-3 protein in the pancreas of model group were augmented,the difference was remarkably(P<0.01);compared with the model group rats,Caspase-3 expression of protein in point embedding therapy group and Caspase-3 expression of protein in point injection group were decreased. the difference was significant(P<0.01); compared with the embedding therapy group,the expression of Caspase-3 protein in pancreas tissues of the point injection group was decreased,the difference was significant(P<0.01). Conclusion1. Point embedding therapy and point injection could decrease blood glucose effectively and improve morphology and secretory function of islet β cell and increase insulin sensitivity. its played a role in the treatment of type 2 diabetes.2. Insulin resistance of Type 2 diabetes mellitus led to the body being in the postprandial hyperglycemia state in a long time,which activated of PANDER gene expression and secretion in the pancreatic beta cell.when the body was in insulin resistance,at the same time,pancreatic beta cell compensatory increased and pancreatic beta cell compensatory prolife ration,which resulted to be increased secretion of insulin. the secretion of PANDER was in the wake of secretion of insulin,secretion of beta cell of islet PANDER levels would be increased,the physiological function of PANDER was to promote islet beta cell to apoptosis,the main mechanism of PANDER induced beta cell apoptosis was activation of Caspase-3 and restrain the plethoric expression of p21.Plethoric expression of PANDER irritated the expression of Caspase-3,an excess of expression of Caspase-3 restrain the expression of p21,which would damage the function of islet beta cells and induce to cell apoptosis,and aggravate he progression to type 2 diabetes mellitus.3. point embedding therapy and point injection could through alleviate hyperglycemia,thereby inhibiting the stimulation effect on pancreatic islet beta cell transcription of PANDER gene,which could reduce pancreatic expression of PANDER,promote the expression of p21,inhibit the expression of Caspase-3,reduce the apoptosis of pancreatic beta cells,which improved the function of islet beta cells,and alleviated the progression of type 2 diabetes mellitus.4.The effect of improving the morphology and function of pancreatic beta cells of point injecting was better than the effect of improving the function of pancreatic beta cells of acupoint catgut embedding therapy.
