Experimental Study On The Effects Of Huo Xue Xiao Zheng Fang On Th1/Th2Shift And Control Of Cell Apoptosis In The Rats With Adenomyosis

Objective:To summurize Professor’s clinical experiment on treating adenomyosis and explore the Thl/Th2shift and control of cell apoptosis mechanism of Huo Xue Xiao Zheng Fang (HXXZF) on the rats with adenomyosis (AM) by observing the changes of inflammatory cytokines and control of cell apoptosis.Methods:Female Wistar rats were used and the AM model was established by surgical operation. Fifty rats were randomly divided into model group, mifepristone group, Huo Xue Xiao Zheng Fang large, medium and small dose groups. The sham operation group and normal group were set as well. The histomorphological changes of the rats’uteri were observed using light microscope. The enzyme-linked immune sorbent assay (ELISA) was applied to measure the concentration of IL-2、IL-4、IL-10and IFN-y in serum of rats in each group. The immunochemistry method was used to detect the expression of Livin and Smac in rats’endometrium tissue. The expression of LivinmRNA and Smac mRNA were measured by using real time quantitative PCR.Results:Experiment1:The ratios of IFN-γ/IL-4and IL-2/IL-10in model group were significantly lower than those in normal group (P<0.01). This result showed that Thl/Th2shift occurred in AM. After treatment, the serous levels of IFN-γ and IL-2in all treatment groups were significantly increased than those in model group, meanwhile the IL-4and IL-10in all treatment group were elevated. The ratios of IFN-y/IL-4and IL-2/IL-10in all treatment groups were significantly increased than those in model group (P<0.01). The effects of large dose group, medium dose group and mifepristone group significantly outweighed the small dose group.Experiment2:Both Livin and Smac protein expressed mainly in the cytoplasm of glandular cells. The expression of Livin proteinin both ectopic and eutopic endometrium was higher compared with in normal endometrium, and the expression in ectopic endometrium was higher than in eutopic endometrium, the expression of Livin protein in the three groups above had significant difference (P<0.05or P<0.01). The expression of Smac protein in normal endometrium was the highest, in both ectopic and eutopic endometrium was lower eompared with in normal endometrium, and the expression in ectopic endometrium was lower than in eutopic endometrium, the expression of Smac Protein in the three groups above had significant differenee (P<0.05or P<0.01). The coefficient correlation of the expression of Livin and Smac in ectopic endometrium was-0.794, showing the significant negative correlation (P<0.01). After treatment the expressions of Livin and Livin mRNA in all treatment group were significantly decreased than those in model group, meanwhile the Smac and Smac mRNA in all treatment group were increased. The effect of large dose group was best, followed by the mifepristone group and medium dose group, with the small dose group in last.Conclusions:1.The imbalance of Th1/Th2cytokine may be related to the pathogenesis of AM.2. Elevated expression of Livin and decressed expression of Smac cooperated with each other lead to the abatement of programmed cell death in ectopic endometrium. The pathology might contribute to the survival of regurgitating endometrial cells into the myometrium and the development of AM.3. HXXZF can regulate the imbalance of immune system, induce programmed cell death in ectopic endometrium.
