The Third Lumber Vertebra Transverse Process Syndrome (TLVTPS) is one of the common and frequently encountered disease in the clinic, and it is typical clinical manifestation is chronic lower back pain with obvious limite to bend forward, and at the top of the third lumber vertebra transverse processThere is an remarkable effect to TLVTPS treated by acupotomy. The post literature study showes that TLVTPS is an advantages disease in clinic of acupotmy. This research use histomorphology methods and modern molecular biology technology to study the analgesia effects of acupotomy to treat the TLVTPS in three aspects. And by this research to clarify the analgesia effects of acupotomy to treat the TLVTPS, so that supply the experimental support to clinic, and to promote the development of acupotomy.ObjectiveAccroding to observe the local change of histomorphology at the third lumber vertebra transverse process, the change of the contents of plasma and muscle 5-HT,SP,NOS,β-EP and the change of the contents of lumbar spinal cord and hypothalamus SP,NOS,CCK-8,β-EP,ENK, and compared with electroacupuncture, we want to clarify the analgesia effects of acupotomy to treat the TLVTPS, so that supply the experimental support to clinic.Method30 rabbits were divided into five groups randomly, normal group, model group, acupotomy group, eletroacupuncture (EA) group, acupotomy plus EA group, each group have 6 animals. We use the method of insert gelatin sponge to the third lumber vertebra transverse process to induce the TLVTPS rabbit model. After induce the model, acupotomy group use the treatment of acupotomy, EA group use the treatment of EA, acupotomy plus EA group use the treatment of acupotomy and EA both. After the treatment, observe the local change of histomorphology at the third lumber vertebra transverse process by light microscope and transmission electron microscope, test the change of plasma and muscle 5-HT,SP,NOS,β-EP contents and the change of lumbar spinal cord and hypothalamus SP,NOS,CCK-8,β-EP,ENK contents by sandwich ELISA method, and to study the analgesia effects of acupotomy to treat the TLVTPS.Result1. The result of the local histomorphology change at the third lumber vertebra transverse process by light microscope and transmission electron microscope showes that acupotomy treatment to TLVTPS model rabbits can lighten the inflammatory response, lighten the inflammatory cell infiltration, and also reduce the abnormal proliferation of collagenous fiber, so that to promot the tissue repair.2. The result of the change of plasma and muscle 5-HT,SP,NOS,β-EP contents by sandwich ELISA method showes that the plasma and muscle 5-HT,β-EP contents and muscle NOS contents have increased significantly after induce the model (P<0.05,P<0.01), the plasma NOS also showes increase trend. Acupotomy/EA treatment can decrease the plasma and muscle 5-HT,β-EP contents and muscle NOS contents significantly (P<0.05,P<0.01), there results suggested that Acupotomy/EA treatment play an important part in peripheral analgesia effects by the benign adjustment of plasma and muscle 5-HT,NOS,β-EP contents. There is no significant difference of plasma and muscle SP contents among 5 groups, and the mechanism need further study.3. The result of the change of lumbar spinal cord and hypothalamus SP,NOS,CCK-8,β-EP,ENK contents by sandwich ELISA method showes that lumbar spinal cord and hypothalamus SP,NOS,CCK-8 contents have increased significantly after induce the model (P<0.05,P<0.01) and the lumbar spinal cord and hypothalamusβ-EP,ENK contents have decreased significantly after induce the model(P<0.05, P <0.01). Acupotomy/EA treatment can decrease lumbar spinal cord SP,NOS contents and hypothalamus SP,CCK-8 contents significantly compared with model group (P<0.05, P<0.01), the lumbar spinal cord and hypothalamusβ-EP,ENK contents have increased significantly compared with model group (P< 0.05, P< 0.01). For the lumbar spinal cord CCK-8 and hypothalamus NOS, Acupotomy/EA treatment do not show a significant change. These result showes that Acupotomy/EA treatment have a benign adjustment on lumbar spinal cord SP,NOS,β-EP,ENK contents and hypothalamus SP,CCK-8,β-EP,ENK contents in TLVTPS model rabbits, Acupotomy treatment and EA treatment have the similar effect, and there is no overlying effect using the two method together. The results suggested that acupotomy treatment can regulation the synthesis and secretion of central neurotransmitter associated to pain, reduce the central nociceptive transmission and hyperalgia in TLVTPS model rabbits, and enhance the effects of central EOP. Acupotomy treatment plays an important part in central analgesia effects.ConclutionAcupotomy treatment can lighten the inflammatory response and reduce the abnormal proliferation of collagenous fiber, so that to promot the tissue repair in TLVTPS model rabbits. At the same time, Acupotomy treatment plays an important part in peripheral and central analgesia effects in TLVTPS model rabbits by the benign adjustment on plasma and muscle 5-HT,NOS,β-EP contents, and the lumbar spinal cord SP,NOS,β-EP,ENK contents and hypothalamus SP,CCK-8,β-EP,ENK contents.
