Discussion Of Specialized Training Model For Master Of Nursing Specialist In Intensive Care Unit

ObjectiveBased on the core competence of master of nursing specialist majoring in intensive care unit should have,to discuss a specialized training system for them.In order to provide a theoretical basis for the training of the high level,high-quality ICU specialist nurses,and to promote development of nursing education and the process of nursing specialization in China.Methods1.Though literature research,theoretical analysis and semi-structured interview methods,to draft the initial inquiry questionnaire,and by pre-experiment to modify the formal inquiry questionnaire.Experts who been from nursing management,nursing education,clinical nursing and clinical medical field were invited to participate in a two-round Delphi study for assessing the importance or appropriateness of each indicators.2.The data were analyzed by Excel2010 and SPSS21.0 statistical software.Using mean,standard deviation expressed the basic situation of experts.The effective questionnaires rate experts expressed the quality of investigation.The degree of expert authority was expressed by academic level of experts,the degree of familiarity and judgment of experts.Using Mj,Cj,Kj,CV and the weight value of the indicators indicated the importance of each index and the concentration of expert opinions,the Kendall 's W coordination coefficient was to indicate the degree of expert coordination.3.Entry into the standard: Mj>3.5,CV<0.25,Cj?50%,Kj?0.2,otherwise modify or delete,and finally forming a complete,reasonable training mode for master of nursing specialist in Intensive Care Unit.Results1.Selecting 27 experts from Beijing,Shanghai,Jiangsu,Guangdong,Shandong,Liaoning,Henan Province,which covered nursing management,nursing education,clinical care and clinical medicine.The average age was 48.22 + 9.42(28~65)years,and the average length of work was 23.15 + 10.15(11~49)years.2.The response rates were 87.9% and 93.1% in a two-round study and authorities coefficient were 0.853 and 0.877,and the Kendall's W were 0.264(c2=806.662,P<0.001)and 0.337(c2=578.489,P<0.001).3.According to the inclusion and exclusion criteria of indicators,in this research,evidence-based nursing practice ability,CV=0.25,Kj=3.7%,and nursing research practice 3 months,CV = 0.33,so would be deleted.According to the opinions of experts,changed the ability of assessment,prevention,reducing the risk of intensive care through interdisciplinary cooperation to the ability of reducing the risk of intensive care;The indicators under the ability of scientific research integrated scientific practice operation ability,and increased the ability of scientific thinking and the application ability of scientific research;In the curriculum,the "nursing theory" was changed to "the theory of advanced nursing practice",and "nursing ethics" was integrated into the relevant laws and regulations of medicine,and increased nursing informatics.The training goal,admission requirements,credits and curriculum,clinical practice arrangement,training mode,quality evaluation and corresponding indicators were established.ConclusionThe reliability and validity of requirements are better by rigorous tests,expert's representative and coordinated are better,expert's response rates and authority coefficient are high.The index system can provide references for cultivation of master of nursing specialist.But with the development of nursing and the change of ICU specialist nurse role function,the training system is still in the process of adjustment.
