| ObjectiveGuided by the Competency Outcomes and Performance Assessment(COPA)model,the study constructed a new training program for master of nursing specialist majored in ICU nursing with the idea of the combining the cultivation of master degree program in nursing with the training of new nurses.The training program that embodies the characteristics of ICU specialties provides new ideas for the cultivation of master of nursing specialist in China.Method1.Literature review: The research group researched the literature databases and checked the relevant documents,policies and training programs for masters of science in nursing training program in medical universities.Based on the results of the literature review,a training program for the masters of science in nursing in ICU was initially drafted.2.Semi-structured interview: In order to deeply improve the contents of the training program,the research group conducted semi-structured interviews with six experts specialized in ICU nursing,nursing education,and nursing management.3.Delphi Method: 30 experts specialized in the related fields were selected from the country for two rounds of consultation,and the experts were asked to rate the importance of each item according to the Likert 5-point scoring method.The index deletion criterion is satisfying the importance assignment <3.5,coefficient of variation>0.25 simultaneously.We modify or delete the items according to the experts' opinion and the score.4.Statistical methods: The general information of the experts is described by the rate,standard deviation and composition ratio.The reliability of the research is expressed by the experts' response rate,experts' authority and experts' coordination coefficient;the importance of each item is analysed by standard deviation and weight value.Result1.The results of literature review: 268 literatures were finally included in this study,and the literatures were divided into two groups according to the degree of correlation.The first group was a group of non-directly related literatures(78 pieces),which mainly included the status quo,countermeasures and development Suggestions,according to the current status and problems of MNS cultivation and lay a foundation for the establishment of a scientific and feasible MNS cultivation scheme in this study.The other group is directly related literature(190 pieces),mainly focusing on the exploration of MNS cultivation content,including cultivation mode,core competence,curriculum setting,clinical practice and integration with the training of new nurses.2.The results of Semi-structured interview: Through the collation and analysis of the interview data,two major themes were determined.One is the feasibility of effective connection between master of nursing specialist training and new nurses training,the other is the training strategies of master of nursing sprcialist in ICU.3.The results of Delphi: 25 experts from 12 provinces completed two rounds of expert consultation.The average age of the expert is(49.56±7.56)years,and the average working experience is(28.32±8.93)years.The experts' s response rate of the two rounds consultation were 93.33% and 89.29%.The expert authority coefficients of the two rounds consultation were both 0.91 and greater than 0.7.The two rounds of Kendall's coordination coefficients were 0.217(P <0.001)and 0.206(P < 0.001)respectively.4.The results of item modification: In the first round of expert consultation,the training program contained 174 indicators,and 12 experts put forward 35 suggestions for revision.According to the inclusion and exclusion criteria of the indicators in this study,a total of 29 indicators were added,4 indicators were deleted,and 12 indicators were modified.In the second round of expert consultation,there were 199 indicators,and 3 experts put forward 5 suggestions for revision.4 indexes were modified and 1 index was added.After two rounds of consultation,the training program for the master of nursing specialist in ICU was established,including the training objectives,curriculum,overall time schedule,clinical practice content,process assessment,graduation and degree award conditions.ConclusionAfter two rounds of consultation with experts,a standardized training program for MNS in ICU direction based on the training of new nurses was established.The response rate and authority of the experts are heavy and the training constructed is certainly scientific and which can provide new idea for the MNS training in China under the background of new nurse training.With the continuous development of nursing education in China and the transformation of the roles and functions of ICU specialist nurses,the content of the program constructed in this study will also be continuously improved and updated. |
