ObjectiveThe study is to investigate core competence and general self-efficacy of clinical nurses in tertiary A general hospitals, and analyze the impacts of demographic variables on core competence and general self-efficacy. Studying the differences between evaluations from nurses themselves and other people, finding the correlation between core competence and self-efficacy, and identifying factors that impact nurse core competence, could provide guidance for theoretical basis for nurse core competence training and promote the continuous improvement of clinical care quality.Methods Four tertiary A general hospitals were selected with convenience sampling method, and then300clinical nurses were selected with simple random sampling method in those hospitals. The nurse general information questionnaire, General self-efficacy scale, and Competency inventory for registered nurse were the primary investigative tools. SPSS18.0statistical software was used for statistical analysis. Descriptive analysis was used to analyze general information of nursing. Levene homogeneity of variance test determined whether the results of self-efficacy and core competencies of nurses with different characteristics had homogeneity of variance, which complied with the homogeneity of variance were compared with analysis of variance (ANOVA), and the differences of Multiple sample Wilcoxon were compared with the Kruskal-Wallis Test (H test). A paired t test was used to analyze whether the differences of self-assessment and peer assessments of core competencies were statistically significant. Core competencies as the dependent variables and statistically significant factors in univariate analysis as independent variables, general linear regression analysis were used to determine the influence of independent variables on the dependent variable. The correlation between general self-efficacy and self evaluations and between which and evaluation from other people of nurse core competencies were analyzed by Pearson correlation coefficient analysis.Results1. The mean(±SD) of general self-efficacy score of sampling clinical nursing staff in Shandong tertiary A general hospitals was25.49±6.17, with a range from11to38.There were statistically significant difference of the mean score among the groups of different age, unit, family support and love extent of nursing career(P<0.05).2. The self-assessment scores of core competence were statistically different among groups of different age, unit, title, employment types, working length, marital status, love nursing career and family support (P<0.05), and the peer assessment scores of core competence were statistically different among groups of different age, unit, the first degree, the highest degree, title, employment types, working length, marital status and family support (P<0.05)3. Self evaluation mean score of core competence of the sampling nursing staff was150.53±40.52, the mean score from evaluated by other people was159.52±27.21, and the score from self evaluation and other people evaluation was statistically different (P<0.05).4. The scores from self evaluation and other people evaluation were both positively related with self-efficacy. As for the self evaluation score, the dimensions of critical thinking/clinical nurse scientific research ability were highly related to general self-efficacy, self evaluation mean score of core competence and other dimensions were moderately correlated with general self-efficacy (P<0.01); regarding to other people evaluation, clinical nurses thinking/scientific research ability, clinical nursing ability, leadership, interpersonal relationship and the score from other people evaluation were moderately related with general self-efficacy,and dimensions of ethical/legal practice, professional development, education/consulting were lowly associated with general self efficacy (P<0.01)5. General self-efficacy, age, unit, employment types, working length and family support are the main factors in clinical nursing core competencies.Conclusions1. The core competence score of the sampling clinical nursing staff was in the middle level, of which the leadership score was the highest, and the professional development score was the lowest. The overall level of core competence score from other people evaluation was higher than that of self evaluation.2. The self efficacy score of the sampling nursing staff was in high level.3. General self-efficacy, age, unit, employment types, working length and family support are the main factors in clinical nursing core competencies and the self efficacy is positively associated with core competence.4. Based on the findings, we put forward that the nursing staff managers should optimize the allocation of human resources, should encourage to heighten academic levels, strengthen the continuous education, uptake the degree of social support, improve the job satisfaction of nurses and enhance self-efficacy level, so as to heighten the core competence of nursing staff and eventually improve nursing work performance and of nursing service quality.
