Satisfaction Study And Clinical Assessment Of Patient With Old Complete Denture

Dentition absence is a common complaint in mouth rehabilitation. Document indicated that the dental miss rate of aged people in our country is 6.98%～14.25%,while the repair rate of those is fairly low, and the majority prosthetic replacement are faulty restoration. Dentition absence will affect many functions of the patients such as appearance, vocalization and masticatory function, which is severely impact the patients'quality of life. As a tradition prosthetic style, complete denture has a comparatively large suitable crowd because of the low price and easily nursing. It is a significant measure in diagnose, treat and health care to improve the patient's quality of life and to promote the patient's physical and mental health.In the biology-psychology-social medical model, patient's subjective satisfaction was considered to be the most immediately evaluation of denture prognosis. Although satisfaction is a subjective evaluation, in a way it was a major criterion to measure the success or failure of the complete denture. Formerly research mostly investigated in the satisfaction of the complete dentures which were made latest to find out the influencing factors which impacted the satisfaction of new dentures, but there is little research in the old complete denture.However, along with the employment, the denture's retention and stabilization function will must be digressive because of the changing of occluding relation caused by abrasion, or the changing in oral physiology caused by the differences in the using custom. Under the interaction of multiple factors, the patients'satisfaction would fall off, and then the patient must request to replace a new one, which means that the denture achieved its service life. This research investigated in the satisfaction of those people, wish to find out the main indexes which were decide the service life and the factors which affect those indexes, and supply the rationale in protective measure to prolong the service life.This research collected 79 patients who visit to our hospital,Changchun AiYa out-patient clinic,Changchun ChunCheng out-patient clinic,department of stomatology from Jilin nosocomium and the patients who lived in Changchun TongDe community from July 2008 to mar 2009.First,we investigate the basic information (including sexuality, age, health, smoking habit, tea habit, the origin of dentition absence,the mechanism who made the denture,the custom of using the denture,the reason of reforming,and the using time of the denture)of the patients who using complete denture by the questionnaire. We investigated the satisfaction in 5 indexes involving appearance, vocalization, mastication, easement, stable and firm, and assessment the quality of alveolar crest and saliva, the degree of dental calculus adhesion to the denture, the extent of the denture border, evaluated the personality and request of the patients. We analyzed the information by spss13.0.Our study collected 79 patients who need replacing their complete denture, of the total there are 45 males which account 57% of the total count and 34 female which account 43.0% of the total count. There are 15 smokers which account 19% of the total count, and 30 tea drinkers which account 38% of the total count. The amount of patients whose denture made in special hospital is 20 that occupied 22.8% of the total, whose denture made in general hospital is 19 that occupied 17.7% , whose denture made in out-patient department is 13 that occupied 25.3% , whose denture made in private clinic is 27 that occupied 34.2%.The finding of the satisfaction questionnaire reveal that the most disaffect index is mastication ability,while the lowest disaffect index is pronunciation. The evaluation in appearance and mastication would be different during the different age groups (P<0.05). The disaffect degree of the patients below 50 years old in appearance index will be higher than the 50-70 age group. The disaffect degree of the patients 50-70 years old will be lower than the group over 70.The disaffect degree of the patients over 70 years old in masticatory function will be obviously higher than the other tow groups.The useful life of complete denture is from 0 to 15 years, while the average lifetime is 4.53 years. The useful life of complete denture which the patients who can use correctly (the correct usage complete denture is wearing it during the daytime, and soaking it in cold water after cleaning) will be longer than the one people cannot use correctly(P<0.05). There is no significant difference between the denture which the patients who wear constantly and the one which the patients who seldom use(P>0.05).The patients whose denture made in the department above provincial degree will use the denture more correctly than the patients whose denture made in the basic degree department, while the useful life of denture is longer. There is no obviously associability between sexuality and the useful life of denture.The result of clinical examination display that the satisfaction of the patient whose alveolar crest be plummy is higher than the patient whose alveolar crest is not so plumy in the appearance and steady aspect. The practical time of the denture which has dental calculus adhere to the denture base is longer than the one there's no calculus adhere obviously, while it will be used for longer time that has a small quantity calculus than the denture has a great quantity calculus. The complete denture whose plat extending sufficiently will be used longer than the one whose plat extending insufficiently, while the disaffect degree in aspect of mastication and steady is lower.The analytic result indicates that the satisfaction's correlation factors of the aspect of appearance involved the age of patient, the shape of alveolar crest and the personality of the patient. The patients who have the younger old, the poorer alveolar crest and the refined personality will have the lower evaluation in the aspect of appearance of old denture. The satisfaction's correlation factors of the aspect of mastication involved the age of patient, the saliva quality, the extending of the denture border, the personality and the requirement of the patient. The one who below 70, have the poorer saliva quality, insufficiency denture border extending,high requirement and frightened personality will have the lower evaluation in the aspect of mastication of old denture. The satisfaction's correlation factors of the aspect of comfort involved smoking habit, tea habit and the personality. The one who have the habit of smoking and tea drinking, and the frightened personality will have the lower evaluation in the aspect of comfort of old denture. The satisfaction's correlation factors of the aspect of stability involved the working pressure,the shape of alveolar crest,the saliva quality and the denture border extending. The one who have the greater working pressure,the poorer alveolar crest,the poorer saliva quality and the insufficiency denture border extending will have the lower evaluation in the aspect of stability of old denture.We also discovered that there are associations among the method of administration, health habit, the doctor's horizon and the denture making organization. The patients who made their complete denture in the department which treatment ability above provincial will used more exactly than those who made their denture in the basic department. The contrivance of denture will be more reasonable and the lifetime of the denture will be longer in the department which treatment ability above provincial.Our study can make a conclusion: the satisfaction of old complete denture will be influenced by the objective factors such as age,the shape of alveolar crest,the saliva quality,the contrivance of denture,and the subjective factors such as working pressure,personality and the requirement. Our conclusion sustained the viewpoint in the past. The consequence also cue that we must intension the management of dental hygiene institution, sublimate the medical treatment capability,think highly of advice for the patient and oral hygiene instruction. By means of those, we can prolong the using lifetime of complete denture.
