Background: Edentulous patients with severe residual alveolar ridge resorption,temporomandibular disorders,and dysfunction of stomatognathic system caused by systemic diseases were excluded in numerous studies about denture restorations.The Oral Health-related Quality of Life of these specific patients needs attention but often be overlooked.Objective: To explore the clinical effect of modifiedf treatment denture and the improvement of Oral Health-related Quality of Life in difficult edentulous cases before complete denture restoration,modified treatment dentures were fabricated to solve their own difficult issues before complete denture restoration,and the effectiveness of treatment was evaluated by self-assessment and clinical evaluation.Methods:1.39 patients who experienced failure restoration of conventional complete denture were included in this study.They were divided into four categories: Class I: occlusal abnormalities,Class II: mucosal abnormalities,Class III: Class I + Class II,Class IV: dysfunction of stomatognathic system caused by systemic diseases.They were treated by modified treatment denture.2.OHIP-EDENT,retention and stability,and jaw relation index were evaluated at the stage of old denture,treatment denture and final denture.3.The scores of OHIP-EDENT,retention and stability were shown as meanąstandard deviation(??ąs).Multivariate comparisons were performed using repeated measures analysis of variance.Trend analysis: measurement data were analyzed by variance,and count data were analyzed by chi-square test.In this study,? = 0.05(bilateral),P <0.05 shows statistically significant difference,and the pairwise comparison requires correction of ?,?-correction = 0.05 / number of comparisons.Software Version: spss: 21.0.Result:1.36 out of 39 patients completed the final restoration successfully with a success rate of approximately 92.31%.2.At the 8th week of treatment,the OHIP-EDENT score was significantly reduced(P<0.05);there was no difference in the OHIP-EDENT scores between the 12 th week of treatment denture and final denture(P>0.05).3.The OHIP-EDENT score was significantly reduced at 4th week in Class I(P<0.05/10),and at the 8th week in Class III and Class IV(P<0.05/10).4.As treatment went on,the socres of retention and stability,and jaw relation index became higher,the scores of OHIP-EDENT became lower.(P<0.05)Conclusion:1.Modified treatment denture is an effective pre-restorative option.It can effectively correct occlusal abnormalities and mucosal abnormalities.2.Modified treatment denture can significantly improve the oral health-related quality of life of patients and keep a stable oral health status from treatment denture to final denture.It was the most obvious improvement of the quality of oral health after treatment in patients with occlusal abnormalities.3.From beginning to the end of treatment,stability and retention got better,jaw position tended to be more stable,and the quality of oral health became better.
