Clinical Evaluation Of Bovine Colostrum Containing Oral Hyaloplasmic Gel On Effect Of Complete Denture In Xerostomia Patients

Xerostomia is an oral clinical symptom caused by multiple factors that mainly include malfunction of saliva gland, systematic disease of the whole body, effects of used drugs, radio-therapy of head and face, trauma and mental disease, ect., which have been currently discovered by the popularity. Among them, reduced secretion of saliva is the chief and direct factor. Xerostomia brings a great deal of unfavorable effects to the edentulous patients with complete dentures, for instance, bad denture retention, pain in mucosa, difficulty in chewing and swallowing, disablement of speaking, which may spoil patients' lives.The followings are some progresses made by conventional treatment of xerostomia. (1) increasing the quantity of saliva, preventing teeth to become decayed and guarding against the infection of candida albicans by medical treatment, pilocarpine, for example; (2) acupuncture and moxibustion treatment not only speeded up the saliva secretion of xerostomia caused by different diseases, but also bettered patients' sense of taste, reduced saliva density, improved sleeping quality and alleviated fatigue; (3) artificial saliva, moist oral gel and special made toothpaste also assistanted the treatment. Artificial saliva and gel are usually employed to alleviate the xerostomia by foreign countries, while the traditional artificial saliva is just keeping medicine in mouth to wash, but bottles hold medicine are too large to carry. Additionally, the lasting time for artificial saliva to alleviate xerostomia is not so satisfactory. Thus, patients need to use it continually. Obviously, this method got lower efficiency.Bovine colostrum is abundant in various growth factors,Lactoferrin and Lactoperoxidase, which play important roles in regulating organism natural immunity, fungus groups in intestine tubes, promoting the growth of stomach and intestine or accelerating the healing of intestine trauma. Latest experiment showed that the bovine colostrum, if taken orally, has great controlling power on the outer parts of the infected digestive intestines, that bovine colostrum can alleviate the damage of stomach and small intestine caused by NSAIDs, that bovine colostrum can enhance the defending ability of human's skin.The Bio-gel with the function of alleviating xerostomia has the following 4 mechanism:(1) lubricative function in patient's dry oral cavity--the gel forms a liquid protective screen, which reduced the saliva evaporation through mucosa to lessen xerostomia; (2) bovine colostrum with sweet taste in gel speeded up the production of saliva; (3) bovine colostrum growth factor has a special lubricative function for the saliva gland, especially for the small saliva gland; (4) bovine colostrum growth factor overall inhibits the multiply of candida albicans, so it reduced oral cavity illness possibility of the xerostomia patients caused by the infection of candida albicans.Newly treatment methods offered to the clinical dentists were tested through the clinical effect of the xerostomia patients with complete denture. The comparison results made between 23 xerostomia patients with complete denture treatment with bovine colostrum and 19 non-xerostomia patients with complete denture show as the followings:(1) The overall satisfaction rate about speech, mastication comfort and retention of mandible denture in comparison with patients 3 months moist gel treatment and before the treatment was improved significantly, but little improvement in the rate of retention of maxillary denture. The overall satisfaction rate after 1 month treatment was higher than that of 1 week, but that remained the same satisfaction level in comparison with 3 months and 1 month.(2) Xerostomia patients'function of saliva excretion was worse than normal people. unstimulated whole exudation rate and exudation of K+, Ca2+, Cl- were significantly lower than normal group, but PH of saliva and exudation of Na+ had no significant difference. After using the gel for 3 months, silent saliva exudation rate and exudation of K+, Ca2+ were increased and became stable, and no difference was found compared with normal group, but Cl- was still lower than normal group.(3) Retention of complete denture of xerostomia patients was bad. No significant difference for retention of maxillary denture was found, while retention of mandible denture was significantly decreased, compared with normal group. Retention of maxillary and mandible denture were improved significantly after using the gel for 2 hour. However, retention of maxillary denture didn't improved further after using the gel for 1 and 3 months, while retention of mandible denture increased significantly.In this study, bovine colostrum gel was used to treat xerostomia patients with complete denture. Results showed that general satisfaction rate was significantly improved, mastication and speech were enhanced, unstimulated whole saliva exudation and related ion were increased to stable, and retention of mandible denture was significantly improved.
