According to the latest research results of WHO,there are more than 350 million people suffering from depression in the world,and the disease burden caused by depression has risen to the second place in the world,and will rise to the first place by 2030.In China,about 90 million people are conservatively estimated to suffer from depression;Among them,the elderly in rural areas are more likely to suffer from depression due to weak economic foundation,poor health and other factors.Depression will not only bring great harm to the mental health of the elderly in rural areas,but also indirectly affect their normal physiological function,aggravate the deterioration of physical health of the elderly in rural areas,and then cause additional consumption of medical resources,so the direct and indirect losses are huge.According to the research evidence from the field of public health,the basic medical insurance is an important factor affecting depression.The basic medical insurance system,as an important part of the basic social security for the elderly in rural China,how much influence will it have on the depression score of the elderly in rural areas? What is the mechanism of this influence? Using CFPS(China Family Panel Studies)2012 and 2018 data,this study empirically tested the effect of basic medical insurance on reducing depression scores of middle-aged and elderly people in rural areas,and verified its potential mechanism,so as to put forward corresponding policy suggestions for reducing depression and improving mental health of middle-aged and elderly people in rural areas.This paper is divided into five chapters to study the impact of basic medical insurance on the depression score of rural middle-aged and elderly people.The second to fifth chapters are the core chapters.Firstly,the relevant literature at home and abroad is summarized and summarized in three parts,and a brief theoretical analysis is made from the basic principles of public goods theory and Maslow’s hierarchical demand theory,Explain theoretically why basic medical insurance can reduce the depression score of rural middle-aged and elderly people,and give normative hypotheses for empirical test;Secondly,it explains the data sources of empirical analysis and the measurement models and methods involved in the full text,uses CES-D scale to measure the depression of rural middle-aged and elderly people,and makes descriptive analysis on the core variables and control variables.The participation and degree of rural middle-aged and elderly people in basic medical insurance,the density distribution of their depression score and other index levels were mastered as a whole,and the chronic disease and widowhood impact were confirmed.The main conclusions of this paper are as follows:(1)based on the panel data and two-way fixed effect model composed of CFPS in 2012 and 2018,it is found that basic medical insurance has a positive effect on reducing the depression score of rural middle-aged and elderly people;(2)Using the method of grouping regression to analyze the heterogeneity,it is found that there are differences in the role of different subgroups;(3)The intermediary effect model was used to find the intermediary effect of family per capita income,and the regulatory effect model was used to find the regulatory effect of chronic disease;(4)Panel probit regression and quantile regression were used to test the robustness of the results,and further discussed the difference of the effect of different depression score groups in the sample on participating in basic medical insurance.The main policy suggestions are as follows:1、It is suggested to improve the level of rural basic medical insurance,improve the overall planning level,reduce their probability of returning to poverty due to illness,reduce their concern about the burden of disease,and reduce their depression score by improving the compensation treatment and scope of compensation for the middle-aged and elderly in rural areas.2、Women,those suffering from chronic diseases and widows are vulnerable groups with depression scores.Among them,the effect of reducing depression scores of women after participating in basic medical insurance is not significant.It is suggested to carry out special subsidies and public welfare activities to improve the utilization rate of their basic medical insurance.3、 For chronic disease groups,improve the "three-level prevention of chronic diseases" and strengthen the management of chronic diseases among middle-aged and elderly people in rural areas.
