Abstract/Summary:
Objective:Based on the theory of the five movements and six qi,Shengming Tang is applied to treat patients with gouty arthritis caused by the luck characteristic of "Shaoyang XiangHuo SiTian and Jueyin Fengmu ZaiQuan":1.To verify the clinical efficacy of Shengming Tang in treating gouty arthritis,and to summarize the main symptoms for which Shengming Tang is suitable,so as to confirm the clinical significance of the theory of the five movements and six qi in the field of bone and joint diseases.The clinical significance of Shengming Tang in the field of bone and joint diseases was confirmed,and the formula selection criteria for this formula were used as a reference;2.The distribution of the main YunQi elements at birth in patients with gouty arthritis of the "HuoYinFengSheng" type was discussed,with a view to providing theoretical references for the early prevention of this disease.Methods:A total of 70 patients with gouty arthritis who attended the outpatient orthopedic department of our hospital from January 2022 to January 2023(RenYin year)and met the screening criteria of this study were selected and divided into a control group and an observation group using the random number table method.The control group was given basic treatment combined with etoricoxib tablets orally,while the observation group was given Shengming Tang orally on this basis for 2 weeks.The patients’simplified McGill pain scale scores,duration of joint pain,erythrocyte sedimentation rate,blood uric acid value and each concurrent symptom and its severity were accurately recorded before and after treatment.1.To verify the efficacy of Shengming Tang by comparing the data between groups,and to compare the evidence scores between groups with high frequency symptoms,so as to summarize the main symptoms for which Shengming Tang is applicable;2.To analyze the actual frequency of the main YunQi elements at birth in all patients The differences between the actual and theoretical frequencies of the main YunQi elements at birth were analyzed to explore the innate endowment characteristics of this group,in order to provide theoretical references for the early prevention and treatment of diseases.All data were statistically analyzed by SPSS25.0.(Ⅵ)For visual dizziness,the efficacy of the two groups was comparable,and both groups were ineffective,with no statistically significant difference(P>0.05);(Ⅶ)The main symptoms for which Shengming Tang is indicated:bitter mouth,irritability,dry mouth,dizziness,dry stools,headache,and distension and pain under the duress.Results:1.The mean scores of three items in the PRI score,VAS score and PPI score in the observation group were lower than those in the control group in the comparison between the two groups of simplified Mc Gill pain scale scores in the acute phase(3d),and the difference was statistically significant(P<0.05).2.The average duration of joint pain in the control group was 9.03 ± 3.64 days,and the average duration of joint pain in the observation group was5.35 ± 2.29 days,and the difference between the groups was statistically significant(P<0.05).3.The blood uric acid value decreased in both groups at 2 weeks of treatment,and the value in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).4.The erythrocyte sedimentation rate decreased in both groups at two weeks of treatment,but the difference between the groups was not statistically significant(P>0.05).5.Comparison of TCM symptom scores at two weeks of treatment :(Ⅰ)The distribution of each high-frequency symptom between groups was tested by chi-square test,and the differences were not statistically significant(P>0.05),and all of them were comparable;(Ⅱ)High-frequency symptoms:bitter mouth(80.95%),irritability(74.60%),dry mouth(68.25%),dizziness(68.25%),dry stool(66.66%),blurred vision(61.90%),headache(57.14%),and hypochondriacal distension(42.86%);(Ⅲ)The control group was ineffective for bitter mouth,irritability,dry mouth,dizziness,dry stool,and visual dizziness,and the difference was not statistically significant(P>0.05)in the before-and-after comparison within the group,but was effective for headache and hypochondriacal distension,and the difference was statistically significant(P<0.05);(Ⅳ)The observation group was effective for bitter mouth,irritability,dry mouth,dizziness,dry stool,headache,and hypochondriacal distension,and the difference was statistically significant(P<0.05)in the before-and-after comparison within the group,but not effective for visual dizziness,and the difference was not statistically significant(P>0.05);(Ⅴ)After treatment,except for visual dizziness,all the symptoms in the observation group were more effective than those in the control group,and the difference was statistically significant(P<0.05);6.Comparison of luck endowment:(Ⅰ)The highest frequency of age luck at birth was too much fire luck in 63 patients,the difference was statistically significant(P<0.05),the remaining 9 items were not statistically significant(P>0.05);(Ⅱ)The highest frequency of distribution of the Qi of Sitian Zaiquan was Shao-yin Jun-huo Sitian/Yangming Zao-jin Zaiquan(28.57%),followed by Jue-yin Feng-mu Sitian/Shaoyang Xiang-huo Zaiquan(26.98%),and the difference was statistically significant(P<0.05)when compared with the theoretical percentage(16.67%),while the remaining four items were not statistically significant(P>0.05);(Ⅲ)None of the main gas distributions were significantly different(P>0.05);(Ⅳ)The most frequent polite appearance was Shaoyang phase fire(28.57%),and the difference was statistically significant(P<0.05),while the remaining five items were not statistically significant(P>0.05).Conclusion:1.In this study,we applied Shengming Tang to treat patients with gouty arthritis whose pathogenic mechanism was "Huo Yin Feng Sheng" based on the theory of the five movements and six qi,it can significantly improve the local pain of joints in the acute stage,shorten the duration of joint pain,and also improve the other symptoms of patients to a certain extent and improve the quality of life of patients,which proves the effectiveness of the theory of the five movements and six qi in the bone and joint clinic,and also summarizes that the main symptoms applicable to Shengming Tang are bitterness of mouth,irritability,dry mouth,dizziness,dry stool,headache,and distension and pain under the daisy.This 7 symptoms can be considered as the reference basis of Shengming Tang.2.Among the YunQi elements at birth,the Sui-Yun Huo Yun Tai Guo,and the SiTian-ZaiQuan is ShaoYin JunHuo Si-Tian/Yang-Ming ZaoJin ZaiQuan or Jue-Yin Feng-Mu Si-Tian/ShaoYang XiangHuo ZaiQuan,and polite ShaoYang XiangHuo,these four kinds of Yun Qi characteristics may be the risk factors for developing gouty arthritis(with " Huo Yin Feng Sheng " as the pathogenesis).
