Research On The Problem Of Integration Of Medical Care And Nursing Care In Rural Z County, Shandong Provinc

With China’s accelerated entry into an aging society,the medical problems and pension issues of the elderly have become the key factors related to the quality of life of the elderly group.In order to better cope with the challenges brought by the aging population,the combination of medical care and elderly care in China arises at the historic moment.At the same time,due to the limitations of natural environment,social resources and their own conditions,the rural elderly people face more difficult problems compared with the urban elderly people.China has introduced targeted measures for the rural areas in the combination of medical care and elderly care.The combination of medical care and elderly care has become an effective way to meet the medical and pension needs of the rural elderly people.Based on the above background,Z County,the model antecedent county of combination of medical care and elderly care in Shandong Province,was selected as a typical area in this study.Based on the theory of healthy aging,the theory of hierarchy of needs and the theory of welfare pluralism,this study uses questionnaire survey method and interview survey method to study the development of the combination of medical care and elderly care in rural areas of Z county.This study summarizes the development background of the combination of medical care and elderly care in rural areas of Z county,including the aging population in Z County and the supply of elderly care and medical resources,and analyzes current situation of the combination of medical care and elderly care in rural areas of Z county,including the formulation of relevant policies,the clarification of departmental responsibilities,the establishment of rural institutions,and the classification of rural service modes.Through the above research,this study summarizes and sorts out the problems of the combination of medical care and elderly care in rural areas of Z county in terms of institutional construction,service capacity and talent team,and analyzes the reasons behind the problems from the three levels of policy,concept and society.Finally,combined with the actual situation of Z County,on the basis of fully referring to the experience and practices of Britain,Japan,Germany,Shanghai,Qingdao and Shenyang in supervision and management,talent training,etc.,suggestions are put forward to promote the development of the combination of medical care and elderly care in rural areas of Z county from four perspectives: system formulation,fund guarantee,service improvement and talent training.The results show that the rural areas of Z County are facing a prominent aging problem,and the pension and medical resources are relatively scarce compared with the cities.It is urgent and necessary to carry out high-quality combination of medical care and elderly care in rural areas of Z county.After being identified by Shandong Province as model antecedent county of combination of medical care and elderly care in 2018,Z County formulated relevant top-level systems,clarified the division of responsibilities of various government departments,set up 10 integrated medical and elderly care institutions in rural areas,and carried out five modes of services: "embedded service" mode,"medical institutions providing elderly care services" mode,"elderly care institutions providing medical services" mode,"two institutions in one" mode,and extended medical and health services to family mode.It is found that the problems existing in the rural combination of medical care and elderly care in Z County mainly include the imperfect construction of rural institutions,the poor ability of rural service and the shortage of professional personnel.The reasons for these problems are not only the lagging system design and outdated ideas,but also the backward rural development.In order to further promote the development of the combination of medical care and elderly care in rural areas of Z county,it is necessary to optimize the policy system,establish a multi-channel fund guarantee system,improve the service capacity of the combination of medical care and elderly care,and attach importance to the construction of professional talents.
