| Purpose:To observe the clinical therapeutic effect of PKP on osteoporotic thoracolumbar compression fractures;to observe the effect of Duhuo Jisheng Decoction on residual back pain in patients with deficiency of both spleen and kidney after PKP of osteoporotic thoracolumbar compression fractures.Material and method:256 patients with osteoporotic thoracolumbar compression fractures treated with PKP in the Department of Orthopedics and Trauma,Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from January 2018 to October 2021 were selected.The VAS score,ODI score and TCM syndrome score of all patients before and2 weeks after operation were recorded and observed.70 patients with low back pain after 2 weeks were selected and divided into Duhuo Jisheng decoction group and control group by random number table method,with 35 cases in each group.The two groups of patients began to take medicine after 2 weeks of operation,and both took medicine for 4 weeks.They were followed up for 18 weeks from the day of operation.During the follow-up,6 cases fell off due to various reasons.Finally,64 patients were included,32 cases in the Duhuo Jisheng Decoction group and 32 cases in the control group.The gender,age,bone mineral density and BMI of the two groups were recorded and observed.The VAS score,ODI score,TCM symptom score,grip strength and paravertebral muscle cross-sectional area of the two groups before and 12 weeks after medication were recorded and observed.Results:1.VAS score,ODI score and TCM symptom score of all patients were decreased after operation and before operation,and the difference was statistically significant(P<0.05).2.There was no significant difference in gender,age,bone mineral density and BMI between the two groups(P>0.05).3.At 2 weeks after operation,there was no significant difference in VAS score,ODI score and TCM syndrome score between the two groups(P>0.05).Twelve weeks after medication,the differences in VAS score,ODI score and TCM symptom score between the two groups were statistically significant(P<0.05).4.There was no significant difference in grip strength and paravertebral muscle cross-sectional area between the two groups at 2 weeks after operation(P>0.05).12 weeks after medication,there was no significant difference in grip strength between the two groups(P>0.05).Twelve weeks after medication,the difference in the cross-sectional area of paravertebral muscles between the two groups was statistically significant(P<0.05).Conclusion:1.PKP is effective for pain and back function in patients with osteoporotic thoracolumbar compression fractures.2.Duhuo Jisheng Decoction can effectively relieve back pain,restore back muscle function and improve the overall situation.3.Duhuo Jisheng Decoction had no significant effect on improving the grip strength of patients,but it had a certain promoting effect on increasing the volume of paravertebral muscle.4.Duhuo Jisheng Decoction may relieve residual back pain by increasing paravertebral muscle volume and delaying sarcopenia. |
