Observation On The Curative Effect Of Duhuo Jisheng Decoction Combined With Acupuncture In The Treatment Of Lumbar Disc Herniation

Objective: Lumbar disc herniation(LDH)has a profound adverse effect on the physical and mental health of patients.It is one of the main causes of lower back and leg pain in the elderly,which greatly affects the quality of life and living standards of patients.This study aims to observe the therapeutic effect of Duhuo Jisheng Decoction(DJD)combined with acupuncture on LDH.Methods: From Jan.2019 to Dec.2020,60 patients with LDH admitted to our hospital were divided into two groups according to the order of admission by random number table method.They were routine acupuncture treatment group(n = 30)and traditional Chinese medicine combined treatment group(n = 30).The conventional acupuncture treatment group was treated with ordinary acupuncture in supine position.The main acupoints were Dachangshu,Zhibian,Shenshu,Weizhong,Guanyuanshu and Huanjuan.Acupuncture once a day,10 days a course of treatment for 2 courses.The traditional Chinese medicine combined treatment group was based on the treatment of the conventional acupuncture treatment group,assisted by oral administration of traditional Chinese medicine decoction.The Duhuo Jisheng decoction was selected,each dose contained 167 g crude drugs,one dose a day,decoction 400 m L,split into morning and evening after meals.Each course of treatment for 10 days,for 2 courses.The differences in VAS,therapeutic effect,Oswestry disability index(ODI),Japanese Orthopaedic Association Score(JOA),imaging indicators,inflammatory cytokines and safety between the two groups before and after treatment were observed.Results: In the conventional acupuncture group,VAS decreased from 4.67 ± 1.03 to 2.20± 1.19,Oswestry disability index(ODI)decreased from 42.83 % ± 10.10 % to 27.10 % ±13.20 %,Japanese orthopaedic association scores(JOA)increased from 16.03 ± 2.57 to23.63 ± 2.63,VAS decreased from 4.27 ± 0.98 to 1.43 ± 0.77,ODI decreased from 41.40 %± 13.90 % to 20.30 % ± 10.10 %.The effect of acupuncture combined with traditional Chinese medicine was better than that of acupuncture alone(P<0.05).There was no significant difference in imaging indexes between the two groups before and after treatment(P > 0.05).There was no significant difference in plasma substance P(SP)before treatment(P > 0.05).After treatment,SP in the combined treatment group was significantly lower than that before treatment(P<0.05).After treatment,β-EP in the combined treatment group was significantly higher than that in the conventional acupuncture group(P<0.05).After treatment,BK in the combined treatment group was significantly lower than that in the latter(P<0.05).After treatment,CRP in the combined treatment group was significantly lower than that in the conventional acupuncture group(P<0.05).Conclusion: Ordinary acupuncture treatment of LDH and acupuncture treatment combined with Duhuo Jisheng Decoction in the treatment of LDH have obvious curative effects in alleviating pain,restoring lumbar function and improving quality of life.Acupuncture treatment combined with Duhuo Jisheng Decoction has statistically significant improvement in effect than acupuncture treatment alone,and there is no safety hazard detected.
