Clinical Observation Of Duhuo Jisheng Decoction Combined With Multimodal Analgesia In Pain Management Of Total Knee Arthroplasty

ObjectiveUp to now,the number of patients with knee osteoarthritis performed total knee arthroplasty is increasing.Postoperative pain management is an important part of rapid rehabilitation.Optimizing analgesia is a powerful condition to promote functional recovery.The concept of multimodal analgesia emerged occure for the requirement.The article discussed the clinical effect of multi-modal analgesia in pain management after total knee arthroplasty,and also studied the effect of Duhuo Jisheng Decoction on pain management after total knee arthroplasty to guide clinical medication.MethodsIn this study,100 patients with osteoarthritis of the knee and unilateral total knee arthroplasty admitted to our hospital from Mar.2018 to Dec.2018 were randomly and comparatively analyzed.According to different treatment schemes,the patients were divided into treatment group(50 cases)and control group(50 cases).Multimodal analgesia was used in the control group.The treatment group was treated with Duhuo Jisheng Decoction on the basis of the control group.The collected data including the age,course of disease,range of motion of joints,Visual Analogue Scale(VAS),Hospital for Special Surgery Knee Score(HSS),Activity of Daily Living(ADL),ESR and CRP were analyzed and compared by t test(independent Sample t test comparison between the two groups).Repeated analysis of variance was used to measure data at different time points in two groups.Fisher ?2 test was used to compare the counting data of sex,right or left,the number of remedial drugs used and the number of adverse reactions.All data were analyzed and processed by SPSS 21.0 software package,based on which,there was statistical difference on the condition of P<0.05 and significant statistical difference in the case of P<0.01.Resu tsA total of 100 patients with knee osteoarthritis and unilateral total knee arthroplasty were enrolled in this study.The age ranged from 55 to 83 years,with an average age of 67.43±6.08,and the course of disease ranged from 3 to 15 years,with an average of 7.27±2.42 years.There were 13 males and 87 females.53 cases were left and 47 cases were right.There were 50 cases in the treatment group and 50 cases in the control group.There was no significant difference in age,sex composition,location and course between the two groups(P>0.05).There was no significant difference in preoperative activity,3th day postoperation activity,5th day postoperation activity and 14th day postoperation activity between the treatment group and the control group(P>0.05).There was significant difference in the 28th day postoperation activity between the two groups(P<0.01).There was no significant difference in the preoperative VAS score,the first day postoperation VAS score and the 5th day postoperation VAS score between the treatment group and the control group(P>0.05).There was significant difference in the 14th day postoperation VAS score and the 28th day postoperation VAS score between the treatment group and the control group(P<0.05).There was no significant difference in preoperative HSS score and 14th day postoperation HSS score between the treatment group and the control group(P>0.05).There was significant difference in HSS score between the two groups on the 28th day after operation(P<0.01).There was no significant difference in preoperative ADL score between the treatment group and the control group(P>0.05).The ADL scores of the patients in the treatment group on the 28th day after operation were significantly different(P<0.05).There was no significant difference in the number of cases of pain remediation and complications between the two groups(P>0.05).There was no significant difference in ESR before operation,on the first day after operation,on the 5th day after operation and on the 14th day after operation in the treatment group(P>0.05).There was no significant difference in CRP before operation,on the first day after operation and on the 5th day after operation in the treatment group(P>0.05),but there was significant difference in CRP on the 14th day after operation in the treatment group(P<0.05).ConclusionThe combination of multi-mode analgesia and Duhuo Jisheng Decoction can effectively reduce the pain of patients after total knee arthroplasty,reduce the incidence of local aseptic inflammation of the affected knee,increase the activity of the knee joint after operation,reduce the use of remedial drugs,enable patients to perform rehabilitation exercises under mild or even painless conditions,promote the recovery of knee function and improve the life quality of patients.Syndrome differentiation and treatment of traditional Chinese medicine can improve the patient's constitution.Perioperative Chinese medicine treatment of invigorating liver and kidney,activating blood circulation and relieving pain can enhance the clinical effect of patient' s analgesia program.
