Evaluation Of The Effectiveness Of Conservative Treatment In Stump VAIN

Objective To explore the clinical efficacy of the Chinese patent medicine Paitling and microwave combined with in the treatment of VAIN that occurs in the vaginal stump after total hysterectomy.Baofukang SuppositoryMethods The clinical data of 100 patients with VAIN diagnosed by colposcopic examination and final histopathological confirmation at our cervical lesion center for cervical disease(CIN,CC)or non-cervical disease after total hysterectomy with abnormal follow-up results(TCT,HPV abnormalities)are collected from Ningxia Medical University Hospital from January 2019 to July 2022.The clinical efficacy of TCT and HPV test results,colposcopic lesion assessment,and histopathological examination of the stump are evaluated using the TCT and HPV test results,colposcopic lesion assessment,and histopathological examination of the stump before and after treatment.The patient’s general clinical data(age,reason for hysterectomy,menopausal status,TCT,HPV,subvaginal lesion images,and stump histopathology results)before treatment,colposcopic assessment of image changes one month after treatment,objective indicators after treatment in both groups from 4 to 6 months after surgery: TCT,HPV test results,and biopsy results if necessary;subjective indicators:post-treatment complications.Results All 100 patients with VAIN stump had no obvious clinical symptoms and were referred for colposcopic biopsy of the severed ends after routine postoperative follow-up for abnormal TCT and(or)HPV of the vaginal stump.Sixty-three of the patients with VAIN were treated with the Chinese medicine Paterol,4 of which were ineffective,but the disease did not progress,22 of which had a decrease in lesion grade,with an overall efficiency of93.65% and a complete conversion rate of 58.73%.In 37 patients treated with microwave combined with povidone-based suppositories,3 cases were ineffective,and 17 cases had decreased lesion grade,with an effective rate of 91.89% and a complete conversion rate of45.95%.One hundred patients with stump VAIN had no systemic symptoms such as fever,chills,and allergy during treatment,while three patients experienced the localized loss of mucous membrane on the lateral wall of the vagina and inner labia majora during treatment with Paterol,with a small number of vesicles The incidence of adverse reactions was 4.76%;5 patients experienced local ulceration,pain,and vaginal drainage during the treatment with microwave combined with povidone-based suppositories,the incidence of adverse reactions was 13.51%,the difference in the incidence of adverse reactions between the two treatment methods was not statistically significant(P>0.05).Conclusion The combination of Chinese patent medicine Paitling and microwave with Baofukang Suppository is effective in the treatment of vaginal intraepithelial lesions in the stump,which is easy to operate and has the characteristics of small trauma and quick recovery,and provides a feasible treatment plan for the conservative treatment of patients with VAIN in the stump,which can be used as a reference for clinicians.
