ObjectiveThrough retrospective analysis of clinical data of inpatients with primary liver cancer in our hospital,this paper explores the related risk factors affecting the occurrence of primary liver cancer,and discusses the similarities and differences of the risk factors of primary liver cancer in young people,male and female groups of primary liver cancer,and summarizes the characteristics and risk factors of the occurrence and development of liver cancer.Enhance people’s cognition level of liver cancer,and provide a theoretical basis for early prevention,early diagnosis and prognosis of liver cancer in the future.MethodsThe terms "primary liver cancer","liver cancer","liver malignant tumor","hepatocellular carcinoma" and other terms were searched in the medical record room system of our hospital,and the time range was limited to January 2018 to December 2021.949 inpatients with primary liver cancer in our hospital who met the diagnostic criteria for primary liver cancer were retrieved as the primary liver cancer group.Based on the primary liver cancer group,patients ≤40 years old were selected as the young primary liver cancer group,all males were selected as the male primary liver cancer group,and all females were selected as the female primary liver cancer group.At the same time,a total of 144,525 inpatients in the same period as the case group were searched,and 128,855 patients were left aiter screening the patients diagnosed with neoplastic diseases.According to the statistical principle of "completely random sampling",which was published in the People’s Medical Publishing House on page 49 of the Fourth Edition of Medical Statistics,according to 1:1.Matching principle:A total of 949 patients with non-primary liver cancer were selected "completely randomly" from 128,855 patients in the medical record Statistics Department of our hospital.Matching conditions:same sex,age±5 years;Based on the non-primary liver cancer group,patients ≤40 years old were selected as the young non-primary liver cancer group,all males were selected as the male non-primary liver cancer group,and all females were selected as the female non-primary liver cancer group.The age,sex,weight,height,body mass index(BMI),smoking history,drinking history,family history of liver cancer and rural/urban areas of primary liver cancer group and non-primary liver cancer group,young primary liver cancer group and young non-primary liver cancer group,male primary liver cancer group and male nonprimary liver cancer group,female primary liver cancer group and female nonprimary liver cancer group were summarized Town residence history,geographical distribution,nationality,diabetes history,hepatitis virus markers(mainly refers to HBV,HCV),UREA,CREA,ALT,AST,GGT,ALB,TBIL,DBIL,the above clinical data were recorded in Excel and sorted into a database,the value of ALB was in line with normal distribution.Other factors,such as ALT,AST,GGT,TBIL,DBIL,UREA and CREA,were selected according to the statistical principle due to skewed distribution.For data analysis,Excel and SPSS 26.0 were used for relevant statistical analysis.For comparison of general conditions and biochemical indicators among groups,t test was used for measurement data,expressed as mean±standard deviation(x±S),and X2 test was used for counting data.Logistic regression model was used to analyze the influencing factors of liver cancer.Take P<0.05,indicating statistically signifiant difference.Results①Among 949 patients with primary liver cancer,there were 792 males(83.46%)and 157 females(16.54%),with a male-female ratio of 5.04:1.The overall age of onset was(58.43±13.03)years old,and the overall age of onset of P25-P75 was 49.5 to 68 years old.The age of onset of male(57.58±12.72)years old was lower than that of female(62.68±13.79)years old.The highest number of cases occurred in the age group of 49.5 to 68.②In the comparison between primary liver cancer group and non-primary liver cancer group,the smoking rate,alcohol consumption rate,family history rate of liver cancer,the prevalence of cirrhosis and diabetes in primary liver cancer group,HBV and HCV infection rate and ALT,AST,GGT,UREA,TBIL,DBIL values were higher than the control group,and the ALB value was lower than the control group.The difference was statistically significant,but there were no statistically significant differences in obesity rate,rural/urban residence history and CREA value between the two groups.Multivariate logistic regression analysis showed that HBV infection,HCV infection,cirrhosis,family history of liver cancer,drinking history,smoking history and diabetes history were the risk factors for primary liver cancer,and the differences were statistically significant.③In the comparison between young primary liver cancer group and young nonprimary liver cancer group,the HBV infection rate,family history rate of liver cancer,cirrhosis prevalence,ALT,AST,GGT,TBIL and DBIL values of young primary liver cancer group were higher than those of control group,and the ALB value was lower than that of young non-primary liver cancer group,the difference was statistically significant.There were no significant differences in smoking rate,alcohol consumption rate,diabetes prevalence rate,obesity rate,rural/urban residential history,UREA and CREA values between the two groups.Multiple logistic regression analysis showed that HBV infection,family history of liver cancer and cirrhosis were the risk factors for primary liver cancer in young adults,and the difference was statistically sipnificant.④In the comparison between male primary liver cancer group and male nonprimary liver cancer group,the smoking rate,alcohol consumption rate,family history rate of liver cancer,prevalence of cirrhosis and diabetes,HBV and HCV infection rate,ALT,AST,GGT,UREA,TBIL and DBIL values of male primary liver cancer group were higher than those of the control group.The ALB value was lower than that of male non-primary liver cancer group,and the difference was statistically significant,while the obesity rate,rural/urban residence history and CREA value between the two groups were not statistically significant.Multivariate logistic regression analysis showed that HBV infection,HCV infection,cirrhosis,family history of liver cancer,drinking history,smoking history and diabetes history were the risk factors affecting the occurrence of male primary liver cancer,and the difference was statistically significant.⑤In the comparison between female primary liver cancer group and female non-primary liver cancer group,HBV infection rate,family history rate of liver cancer,high prevalence of cirrhosis and diabetes,ALT,AST,GGT,TBIL and DBIL values of female primary liver cancer group were higher than those of control group,and ALB values were lower than those of female non-primary liver cancer group,the difference was statistically significant,There were no significant differences in smoking rate,alcohol consumption rate,obesity rate,rural/urban residential history,UREA and CREA values between the two groups.Multivariate logistic regression analysis showed that HBC infection,family history of liver cancer and history of cirrhosis were the risk factors affecting the occurrence of primary liver cancer in womenl and the difference was statistically significant.Conclusion1.The risk factors of primary liver cancer and male primary liver cancer are HBV infection,cirrhosis,HCV infection,family history of liver cancer,drinking history,smoking history and diabetes history,among which the top three OR values were HBV infection,cirrhosis and HCV,and the main risk factors of primary liver cancer were HBV infection.The number of males with primary liver cancer was much more than that of females,and the average age of onset of primary liver cancer in males(57.58±12.72)was lower than that in females(62.68±13.79),and the age distribution of primary liver cancer was concentrated in the age range of 49.5-68 years old.2.The risk factors for female primary liver cancer were cirrhosis,family history of liver cancer,and HBV infection,among which the top three OR values were cirrhosis,family history of liver cancer,and HBV infection,and the main risk factor for female primary liver cancer was cirrhosis.4.The risk factors of primary liver cancer in young people are HBV infection,cirrhosis and family history of liver cancer,among which the top three OR values are HBV infection,cirrhosis and family history of liver cancer,and the main risk factors of primary liver cancer in young people are HBV infection.
