AIMS:With the change in lifestyle, diet structure and the improvement of livingstandards,The incidence of non-alcoholic fatty liver disease showed an increasingtrend.Non-alcoholic fatty liver disease can promote the occurrence and developmentof primary liver cancer,The purpose of this paper is investigate risk factors andclinical features of the patients with non-alcoholic fatty liver who develop primaryliver cancer, provided prevention and prognosis of guiding significance to primaryliver cancer with non-alcoholic fatty liver.Identification of the disease cause ofprimary liver cancer etiology is good at treatment preventionand prognosis.METHORDS:A cross-sectional retrospective analysis of research methods to investigate(2010.1-2013.1) three years1010cases of patients is diagnosed with non-alcoholicfatty liver disease with primary liver cancer.Selected98patients with non-alcoholicfatty liver disease and140patients with HCV from primary liver cancer, analysis ofthe patients clinical features and risk factors. analysis pathology feature of35cases ofpatients with liver pathology in98patients,clinical data and laboratory parametersderived from the first treatment date.use Beck-man automatic biochemical analyzerserum biochemical indicators. The application SPSSl7.0statistical software forstatistical analysis, measurement data using the percentile test,Count data use theChi-square test and rank sum test. Univariate analysis using the log rank test, andmultivariate analysis using Cox proportional hazards models, only statistical singlefactor significant variable eventually into the Cox model,P <0.05was consideredstatistically significant.RESULTS:In1010cases of primary liver cancer patients including140cases of hepatitis Cantibody positive patients, accounting about13.9%of the total number;98cases ofnon-alcoholic fatty liver disease patients, accounting about9.7%of the total number; And incidence of non-alcoholic fatty liver disease with primary liver cancershowed increase trends.Selected98patients with non-alcoholic fatty liver diseasefrom primary liver cancer, male patients59cases accounted for60.2%of the98case,female patients39cases accounted for39.8%of the98case, the ratio of male tofemale is1.5:1;Proportion of the number of male patients with hypertension lowerthan women (P <0.05), statistically significant. The median BMI is26.3kg/m2;Obesitynumber is60cases accounted for39.8%of the98case (BMI≥25.0kg/m2),Thediabetes number is51cases accounted for52.0%of the98case,dyslipidemia,andhypertension number respectively is27cases and54cases accounted for27.6%and55.1%of the98case.The median HDL-C、AFP of Male patients,is lower than themedian HDL-C、AFP of female,(P <0.05),statistically significant. The median tumorsize is6.45(25percentile,5.2;75th percentile,8.6), average is6.3cm.large parts of sizeof tumor in patients is4-8cm,66patients(81%) had single tumor.Child-pughclassification mostly B grade, and C grade, only a small part of the A grade. There isobvious differences in the fibrosis extent of liver tissue pathological features of35patients with non-alcoholic fatty liver disease patients who develop primary livercancer between with male and female, female patients more serious than males in theextent of liver fibrosis, both significant differences (P <0.05), statisticallysignificant. The group of non-alcoholic fatty liver disease patients and group of HCVinfection patients on age, BMI, the number of diabetes, smoking and alcoholconsumption, significant difference P <0.05, statistically significant. Conclusion:The number of patients of nonalcoholic fatty liver disease who developprimary liver cancer share a higher proportion in the total number of primary livercancer, and showed an increasing trend in the proportion..The most number of patientof non-alcoholic fatty liver disease with primary liver cancer is males. The mosenumber of patient of non-alcoholic fatty liver disease who develop primary livercancer with obesity, hypertension, diabetes or dyslipidemia. The number of the malepatients with obesity, diabetes or dyslipidemia is more than female, The number of the femalepatients with hypertension is more than male. The median HDL-C, AFP of Male patients islower than female’s.Compared with women, the development of non-alcoholic fattyliver disease with primary liver cancer in male patients is lest inflammation in the liver fibrosis degree than in female patient.Compared with HCV infectionpatients,non-alcoholic fatty liver disease patients with higher age, BMI, the numberof people with diabetes are more. Old age, alcohol consumption are independent riskfactors to promote non-alcoholic fatty liver disease development to primary livercancer.
