Objective: To examine and evaluate the clinical effectiveness of the acupuncture technique’Golden Hook Fishing’ in the preparation of post-stroke subluxation of the shoulder.To compare and investigate the intervention of the acupuncture treatment method ’Golden Hook Fishing’ in the treatment of post-stroke shoulder subluxation and thus provide a baseline for the clinical implementation of acupuncture treatment.Methods: Eighty patients with a semi-dislocated shoulder joint after stroke were randomly divided into an acupuncture "gold hook fishing" group and a conventional acupuncture group,with 40 cases in each group.In both groups,the usual basic treatment such as basic medical treatment,good positioning of the limb and rehabilitation exercises were applied.The conventional acupuncture group selected the four acupuncture points of "Scapulder,Shoulderium,Tianzong and Bingfeng" as the main acupuncture points for general acupuncture treatment,while the "Golden Hook Fishing" acupuncture group carried out "Golden Hook Fishing" acupuncture treatment on the main acupuncture point of the conventional acupuncture group.The frequency of treatment in both groups was 1/d,6times/week,a total of 4 weeks.Improvements in upper extremity function in patients with hemiluxation of the shoulder joint after stroke,as well as shortest-distance injury(AHI),neuroelectrophysiology,upper extremity motor function(FMA-UE),and upper extremity muscle strength were observed in both groups to monitor and evaluate the clinical effects of golden hook fishing acupuncture in the treatment of this condition.Results:1.Shortest distance between the acromion and the humeral head(AHI): the AHI decreased in comparison with the previous AHI in tandem with the treatment in both groups(p<0.05).When comparing the AHI among the two groups after treatment,the "Golden Hook Fishing" acupuncture group achieved a better correction of the AHI ratio than the conventional acupuncture unit,with a statistical value change(P < 0.05).2.Neurophysiology: The latency and CMAP amplitudes of the suprascapular and axillary nerves in both groups improved to different degrees after treatment(P < 0.05).After the comparison of the latency and CMAP magnitude between the two categories,the "golden hook fishing" acupuncture group was inferior to the conventional acupuncture group in enhancing the latency and CMAP magnitude of the nerves,which showed a statistically considerable variation(P<0.05).3.The Fugl-Meyer Upper Extremity Exercise Scale(FMA-UE): The FMA-UE values improved in both groups after treatment(P<0.05).When FMA-UE scores were compared with each other among the two groups in terms of improvement in upper extremity motor function,the "Golden Hook Fishing" acupuncture group outperformed the conventional acupuncture unit,indicating a tallied change(P<0.05).4.Muscle strengthening assessment: The muscular force of the shoulder was elevated in tandem with that in pre-treatment in each of the above arms in both groups(P<0.05).When comparing the ratings of muscle strength among the two groups after therapy,the "Golden Hook Fishing" acupuncture group was better as compared to the conventional acupuncture group in terms of improving the muscle strength of anterior flexion,posterior extension and abduction of the rotator joint,with statistical data available(P<0.05).Conclusion:1.The Golden Hook acupuncture group and the conventional acupuncture group are both effective in the treatment of lateral shoulder dislocation after stroke,but the Golden Hook acupuncture group is superior to the conventional acupuncture group in terms of overall clinical effectiveness.2.The Golden Hook acupuncture method can effectively improve neurophysiological markers,increase the strength of the periacetabular muscles,correct malalignment of the scapula,reduce the degree of dislocation,increase the stability of the shoulder joint,correct incorrect motor patterns and promote recovery of the motor function of the shoulder joint.
