Objective: To investigate the clinical efficacy of combining rehabilitation training with Jiawei banxiahoupu decoction to treat post-stroke depression of Phlegm Turbidity and Blood.Methods: A total of 80 patients with post-stroke depression with phlegm-turbidied-stasis type admitted to the Department of Encephalopathy in the First Affiliated Hospital of Gansu University of Traditional Chinese Medicine from January 2022 to December 2022 who met the diagnostic criteria were randomly divided into control group and experimental group.Both groups were given basic treatment for post-stroke depression.The control group was given oral paroxetine hydrochloride tablets at the same time of basic treatment,and the experimental group was given Jiawei banxiahoupu decoction on the basis of control group.The changes of TCM syndrome score,HAMD score,NIHSS score,ADL score,PSQI score,blood Hcy,plasma hs-CRP and serum cortisol levels were compared between the two groups before and after treatment.After 4 weeks of treatment,Jiawei banxiahoupu decoction with Chinese herbal was stopped,and follow-up was conducted 4 weeks after treatment,and evaluation of each scale was conducted again.Results: Before treatment,there were no statistically significant differences in the basic data,scoring scales and laboratory indicators between the two groups(P > 0.05),indicating comparability.1.After treatment,the overall effective rate of TCM syndrome scores between the two groups was 89.5% in the experimental group and 73% in the control group,and the Chi-square test showed a statistically significant difference(P < 0.05).The overall effective rate of HAMD score was 86.9% in the test group and 75.8% in the control group,and the Chi-square test showed that the difference was statistically significant(P < 0.05).2.After treatment and follow-up after drug withdrawal,the TCM syndrome score,HAMD score,NIHSS score,and PSQI score in the test group decreased significantly,while the ADL score increased significantly.Compared with the pre treatment and control group,the difference was statistically significant(P<0.05);In the control group,the TCM syndrome score,HAMD score,and NIHSS score decreased after treatment and after drug withdrawal,while the ADL score increased,with a statistically significant difference compared to before treatment(P<0.05).However,the decrease in the PSQI score in the control group after treatment and after drug withdrawal was not significant,with no statistically significant difference within the group(P>0.05)3.Compared with the control group,serum Hcy,plasma hs-CRP and plasma cortisol levels of experimental groups were decreased after treatment,and the comparison between groups before and after treatment was statistically significant(P < 0.05).Correlation analysis showed that blood Hcy level(r=0.239,P=0.039),serum hs-CRP level(r=0.243,P=0.036),plasma COR level(r=0.377,P=0.001)were positively correlated with HAMD score.4.There were no major adverse events during treatment between the two groups,and there was no statistical difference in safety indexes before and after treatment.Conclusion:1.Jiawei Banxia Houpu Decoction combined with paroxetine hydrochloride has a significant clinical efficacy in the treatment of post stroke depression due to phlegm stagnation,significantly improving the patient’s depressive mood,neurological deficits,daily living ability,and sleep status,and sleep status,with good safety.2.Jiawei Banxia Houpu Decoction can reduce the levels of homocysteine,hypersensitive C-reactive protein and plasma cortisol in the patients with phlegm-turbidity and blood-stasis post-stroke depression.Homocysteine,hypersensitive C-reactive protein and plasma cortisol levels were linearly correlated with HAMD scores,which could be used as test indicators to evaluate the effect of post-stroke depression treatment.
