| Objective Through the study of the law of TCM constitution distribution of patients with thyroid nodules and its internal relationship with related factors,to provide clinical basis and diagnosis and treatment ideas for the prevention and treatment of thyroid nodules and the conditioning of people prone to biased constitution.Methods Patients with thyroid nodules who meet the inclusion criteria were selected as the investigation objects,and a questionnaire survey was conducted to collect the basic information of patients,laboratory tests and TCM constitution types.Use Excel software and SPSS26.0 software to process data,and make statistical analysis and discussion.Results1.Among the 254 patients with thyroid nodules,there were 39 males(15.35%)and 215 females(84.65%).The proportion of females was higher than that of males,and the Han nationality was more common in people aged 40 to 60 years old.2.The TCM constitutions of patients with thyroid nodules in Kunming area are mainly yang deficiency constitution,qi stagnation constitution and mild constitution,while the male constitution is mainly yang deficiency constitution,while the female constitution is mainly distributed in yang deficiency constitution,qi stagnation constitution and mild constitution.3.According to the statistics of thyroid nodules,there are 50 cases of cystic,84 cases of solid and 120 cases of mixed cystic and solid.According to the statistics of single/multiple nodules,there are 94 single nodules and 160 multiple nodules.According to the statistics of nodule TI-RADS classification,there are 2 people in TI-RADS1,48 people in TI-RADS2 and 204 people in TI-RADS3.4.Comparison of BMI,course of disease and age of patients with different types of TCM constitution;The BMI of patients with phlegm-dampness constitution was higher than that of other constitutions,and the BMI of patients with special constitution was lower than that of other constitutions,with statistical difference(P<0.05).The course of disease of patients with phlegm-dampness constitution is shorter than that of other constitutions,and the course of disease of patients with special constitution is longer than that of other constitutions,with statistical difference(P<0.05).The patients with blood stasis constitution and qi deficiency constitution are older than other constitutions,while the patients with damp heat constitution constitution,mild constitution,Qi stagnation constitution and special constitution are younger than other constitutions,with statistical difference(P<0.05).5.Comparison of A-function indexes of patients with different types of TCM constitutions: The serum TSH level of patients with blood stasis constitution,phlegm-dampness constitution and qi-deficiency constitution is higher than that of other constitutions,and the serum TSH level of patients with special constitutions is lower than that of other constitutions,with statistical difference(P<0.05).There was no statistical difference in serum T3,T4,FT3,FT4,TGII,TPOAb and TGAb levels among patients with different TCM constitutions(P>0.05).6.Comparison of blood lipid indexes of patients with different types of TCM constitutions: The serum CHOL and LDL-C levels of patients with blood stasis constitution,special constitution and phlegm-dampness constitution are higher than those of other constitutions,while the serum CHOL and LDL-C levels of patients with qi deficiency constitution and qi stagnation constitution are lower than those of other constitutions;The serum TG level of patients with phlegm-dampness constitution was higher than that of other constitutions,and the serum TG level of patients with mild constitution and special constitution was lower than that of other constitutions.The levels of serum CHOL,LDL-C and TG in patients with different TCM constitutions were statistically different(P<0.05).There was no statistical difference in serum HDL-C levels between patients with different TCM constitutions(P>0.05).7.Comparison of uric acid and urinary iodine levels of patients with different TCM constitutions: The uric acid level of patients with phlegm-dampness constitution is higher than that of other constitutions,and the uric acid level of patients with qi stagnation constitution and mild constitution is lower than that of other constitutions,with statistical difference(P<0.05).There was no statistical difference in urinary iodine level between patients with different TCM constitutions(P>0.05).8.Comparison of nodule nature,nodule single/multiple,nodule classification among different types of TCM patients: There was no statistical difference among the groups(P>0.05).9.The correlation between the constitution types of thyroid nodules and the indexes:(1)The age differences between the constitutions of qi deficiency,yang deficiency,yin deficiency,blood stasis and mild were statistically significant(P<0.05).The patients with qi deficiency,yang deficiency,yin deficiency and blood stasis were older than mild.(2)The difference of BMI between phlegm-dampness constitution and mild constitution was statistically significant(P<0.05),and the BMI of patients with phlegm-dampness constitution was greater than that of mild constitution.(3)The differences of serum TSH between Qi deficiency constitution,phlegm-dampness constitution,Qi stagnation constitution,blood stasis constitution and mild constitution were statistically significant(P<0.05),and the possibility of serum TSH increase of Qi deficiency constitution,phlegm-dampness constitution,Qi stagnation constitution and blood stasis constitution was obviously higher than that of mild constitution.(4)The difference of serum TPOAb between Qi-stagnation constitution and mild constitution was statistically significant(P<0.05),and the possibility of serum TPOAb of Qi-stagnation constitution was obviously higher than that of mild constitution.(5)The difference of serum CHOL,TG and uric acid between phlegm-dampness constitution and mild constitution was statistically significant(P<0.05),and the possibility of serum CHOL,TG and uric acid in phlegm-dampness constitution was obviously higher than that in mild constitution.(6)The difference of HDL-C reduction between Qi-deficiency constitution and mild constitution was statistically significant(P<0.05),and the possibility of serum HDL-C reduction in Qi-deficiency constitution was higher than that in mild constitution.(7)There was no significant difference in serum T3,T4,FT3,FT4,TGII,TGAb,LDL-C between normal constitution and other constitutions(P>0.05).Conclusion In Kunming,thyroid nodules are more common in the Han nationality aged40 to 60 years old,and there are more females than males.The TCM constitutions of patients are mainly distributed in Yang deficiency constitution,Qi stagnation constitution and mild constitution.The TCM constitution types of patients with thyroid nodules may be different due to the influence of BMI,course of disease,age,TSH,CHOL,TG,LDL-C,HDL-C and uric acid.Comparing the ages between groups,the patients with blood stasis constitution and qi deficiency constitution are older than other constitutions,while the patients with damp heat constitution,mild constitution,special constitution and qi stagnation constitution are younger than other constitutions.Compared with other constitutions,BMI of patients with phlegm-dampness constitution was higher,while that of patients with special constitution was lower.Comparing the course of disease between groups,patients with special constitution are longer than other constitutions,and patients with phlegm-dampness constitution are shorter than other constitutions.Compared with other constitutions,the serum TSH level of patients with blood stasis constitution,qi deficiency constitution and phlegm-dampness constitution was higher than that of other constitutions,while that of patients with special constitution was lower than that of other constitutions.Compared with other constitutions,the levels of CHOL and LDL-C in serum of patients with blood stasis constitution,phlegm-dampness constitution and special constitution were higher,while those with qi stagnation constitution and qi deficiency constitution were lower.Compared with other constitutions,the serum TG level of patients with phlegm-dampness constitution was higher,while patients with special constitution and mild constitution were lower.Compared with other constitutions,the serum uric acid level of patients with phlegm-dampness constitution was higher,while those with qi stagnation constitution and mild constitution were lower. |
