Objective:To explore the connotation,structure and constituent elements of the competency of nurses specialized in organ transplantation,initially establish a systematic,scientific and feasible evaluation system for the competency of nurses specialized in organ transplantation,and determine the weight of each index.It provides objective and reasonable basis for each process of human resource management,such as the selection,appointment,assessment,training and development of specialized nurses for organ transplantation,so that managers can realize the "person-post matching"according to the basis.It can also provide guidance for the establishment of training courses for nurses specializing in organ transplantation and promote the development of the industry.Methods:Using phenomenological research methods,20 experts from different departments of organ transplantation center of a hospital in Shandong Province were selected as interview subjects through objective sampling.Semi-structured interview was conducted and the data were analyzed by Colaizzi’s 7-step method of phenomenological research.Based on the "competency model","role theory" and"holistic nursing theory" as the theoretical framework,combined with the literature review and qualitative research results,the draft of the correspondence questionnaire was formed to construct the competency evaluation index system of nurses specialized in organ transplantation.The Delphi method was applied to invite 16 domestic experts to conduct two rounds of expert letter consultation,and the analytic Hierarchy process(AHP)was applied to determine the weights of indicators at all levels,and the competency evaluation index system of nurses specialized in organ transplantation was constructed.Findings:①This study refines the theme of competency of nurses specialized in organ transplantation through qualitative interviews with 20 experts.②A total of 16 experts participated in two rounds of Delphi expert letter consultation.Through the statistical analysis of the general information of experts and the degree of authority of experts,it is found that the experts included in this study have good representativeness and authority;The recovery rates of the two rounds of correspondence inquiry were 88.9%and 100%,respectively.The expert authority coefficient(Cr)was 0.972,indicating that the authority of experts was high.After two rounds of correspondence,the coefficient of variation(CV)was as follows:first-level index 0~0.34,second-level index 0~0.14,third-level index 0~0.14,Kendall harmony coefficient(W)was 0.225 and 0.203,respectively,with P values less than 0.00 1,indicating a high degree of expert opinion coordination.Finally,a competency evaluation index system for organ transplant nurses has been formed,which includes 4 first-level indexes,14 second-level indexes and 55 third-level indexes.③ The weights of each level were determined by AHP.The weights of the first level were professional knowledge(0.332),professional operation skill(0.286),professional development ability(0.260)and personal professional quality(0.122).The CR values of all levels of indicators were less than 0.1,the consistency test was satisfactory,and the weight of each indicator was set reasonably.Conclusions:Based on literature review,qualitative research and Delphi method,this study established a scientific,systematic and practical competency evaluation index system for the post of nurses specialized in organ transplantation,including 4 first-level indexes,14 second-level indexes and 55 third-level indexes.And through the analytic hierarchy process to determine the weight of each level of indicators.The experts in this study have good enthusiasm,authority and coordination of opinions,the expert opinions are highly consistent,the weight of each index is reasonable,and the research results are scientific and credible.It provides a reference basis for the evaluation,assessment and appointment of nurses specialized in organ transplantation,and will help the development of organ transplantation nursing. |
