Effect Of Zhitong Rushen Decoction On Postoperative Pain And Inflammatory Indicators In Patients With Mixed Hemorrhoids Pattern Of Damp-heat Pouring Downward

Objective: To determine the effect of Zhitong Rushen Decoction on postoperative pain and serum inflammatory indicators in patients with mixed hemorrhoids pattern of damp-heat pouring downward.Methods: A total of 88 patients who were hospitalized and underwent surgical treatment due to mixed hemorrhoids in the Department of anoenterology of Urumqi city traditional Chinese medicine hospital from December 2021 to December 2022 were collected,and the patients were divided into two groups according to the random number table method,including 44 cases in the control group and 44 cases in the treatment group.Patients in both groups were treated with surgery after admission with relevant preoperative preparation,while patients in the control group were treated with the conventional basic treatment after surgery of mixed hemorrhoids,patients in the control group were treated with local wound changing medicine based on the conventional treatment after surgery,patients in the treatment group were treated with oral traditional Chinese medicine decoction for Zhitong Rushen on the basis of the treatment plan of patients in the control group,and the TCM syndrome scores were compared between the two groups at the pre-treatment,7 days and 14 days of treatment VAS pain score and serum CRP and TNF-α、IL-6 levels,and the clinical efficacy and total response rate after14 days of treatment were compared between the two groups based on the TCM syndrome score.Results:(1)baseline data: there were no significant differences in gender distribution,mean age and mean disease duration between the two groups(P>0.05),and the baseline data were comparable between the two groups;(2)Repeated measures indicated significant differences in TCM syndrome scores,VAS pain scores and serum CRP、IL-6 between the two groups,time point effects,interactions,and between group effects were found(P<0.05),Serum TNF-α in both groups Levels present only a time point effect(P< 0.05);(3)TCM syndrome score: the pairwise comparison before and after treatment within the group showed that the TCM syndrome score of the patients in both groups decreased(P<0.05)compared with that of the patients before treatment,and the score of the patients in the treatment group changed more significantly than that of the control group during the same period(P < 0.05);(4)Clinical efficacy: the two groups differed in the distribution of clinical efficacy(P<0.05),and the 95.00% clinical overall response rate in the treatment group was significantly higher than the 77.50% in the control group(P<0.05);(5)VAS pain scores: within group before and after pairwise comparisons showed that at 7 and 14 days after treatment,VAS pain scores in both groups were reduced compared with those before treatment(P<0.05),and the changes in the treatment group were more obvious than those in the control group during the same period(P < 0.05);(6)Serum CRP,TNF-α、IL-6 levels: within group before and after pairwise comparison showed that 7 and 14 days after treatment,serum CRP,TNF-α and the levels of IL-6 were all decreased compared with those before treatment(P<0.05),and the levels of CRP and TNF-α were similar between the treated group and the same control group at7 days of treatment,there was no significant difference in the comparison of IL-6 levels(P>0.05),but the decrease in serum CRP、IL-6 levels was more obvious in the treatment group after 14 days than the control group during the same period(P<0.05);(7)Neither group of patients experienced any adverse reactions during the group period.Conclusion:Pain relief such as Zhitong Rushen Decoction can effectively relieve postoperative pain and reduce serum CRP and TNF in patients with mixed hemorrhoidal Pattern of Damp-heat Pouring Downward CRP、TNF-α、IL-6 levels,improve the clinical diagnosis and treatment effect.
