| Objective: To understand the basic situation of medical service performance of tertiary hospitals in Xinjiang based on DRG management ideas and clinical big data analysis,compare the differences between traditional indicators and DRG indicators in medical service performance evaluation,and comprehensively evaluate the medical service performance of 12 tertiary hospitals based on DRG indicators.From the perspective of medical staff,to understand the medical staff’s awareness and satisfaction with the implementation of DRG,To explore the application value and specific effect of DRG after its implementation,and provide reference for hospital fine management and subsequent implementation of DRG.Methods: The first page of medical records and DRG grouping data of discharged patients from 26 MDC groups in 12 tertiary hospitals in Xinjiang from2020 to 2021 were collected.Understand the basic situation of medical services in 12 hospitals through descriptive analysis;The TOPSIS and RSR methods were used to evaluate and compare the comprehensive performance of medical services of 12 hospitals based on traditional indicators and DRG indicators;Analyze the correlation between CMI value and traditional indicators through grey correlation model;Through a questionnaire survey,the current status of awareness and satisfaction after the implementation of DRG is summarized.Results:(1)In a total of 12 hospitals included in the study,1279689 cases,the enrollment rate was 98.92%.According to the results,the number of discharged patients in 12 tertiary hospitals reached 744,000,DRG reached 5856,and the PMI value in 5 hospitals was 0.833.In terms of service efficiency,the average hospital cost of 12 hospitals is 13815.24 yuan,the average drug cost is 2823.09 yuan,and the average consumables cost per time is 2288.91 yuan.The average hospitalization day of 7 hospitals is 9.08 lower than the average level9.08,the time consumption index of 6 hospitals is lower than the average level 1.08,and the cost consumption index of 5 hospitals is lower than the average level of 0.98.According to the overall situation of medical safety,the total number of deaths is 3817,671 postoperative deaths,the number of neonatal deaths is 19,and the number of low-risk deaths is 4.Compared with 2020,the medical services of tertiary hospitals basically show a good development trend.(2)The comprehensive performance evaluation results show that there are obvious differences between the comprehensive performance evaluation results of traditional indicators and DRG indicators.Hospital I has the largest difference,with TOPSIS ranking down by 8 places,RSR ranking down by 5 places,followed by Hospital B,TOPSIS ranking down by 8 places,RSR ranking up by 4 places,and other hospitals ranking has no obvious difference.Compared with the CMI value of each hospital,it was found that the ranking of DRG indicators in five hospitals with CMI value greater than the average value of 0.8327 was ahead of the ranking of traditional indicators;On the contrary,the ranking of DRG indicators of 7 hospitals with CMI value less than 0.8327 is lower than that of traditional indicators.(3)According to the results of grey correlation analysis,the correlation between the average length of stay and the CMI value is the highest.(4)Based on the comprehensive performance evaluation results of DRG,hospital C ranks first,followed by hospital A,and hospital F ranks last.Among the specialized hospitals,G hospital ranks first,followed by H hospital,and K hospital ranks last.(5)The results of the questionnaire showed that from the perspective of the recognition of medical staff,the recognition of DRG was 76.05%,which was acceptable;The impact rate is 65.65%.In terms of satisfaction,the satisfaction rate was 78.43%.(6)The results showed that medical staff with different professional titles and degrees varied(P <0.05).The satisfaction of medical staff with different positions,professional titles and academic qualifications varied(P <0.05).It is also considered that professional title is the primary factor affecting the recognition and satisfaction of medical staff on DRG.Conclusion: The medical service situation of tertiary hospitals in Xinjiang is basically showing a good development trend compared with that in 2020.Compared with traditional indicators,DRG indicators have more advantages in the comparability and scientificity of medical service performance evaluation,and the CMI value is considered to be the key indicator that causes the difference between the evaluation results of traditional indicators and DRG indicators.The medical staff’s recognition of DRG is fair,but their satisfaction is low.It is suggested that the hospital should further optimize it according to the actual situation,promote the implementation of DRG,and promote the improvement of medical services and performance of tertiary hospitals. |
