The Relationship Between RDW,MPV And NT-proBNP Of Chronic Obstructive Pulmonary Disease Associated Pulmonary Hypertension

Objective To investigate the correlation between the level of red blood cell distribution width(RDW),mean platelet volume(MPV),and serum N-terminal probrain natriuretic peptide(NT-proBNP)and pulmonary hypertension(PH)secondary to chronic obstructive pulmonary disease(COPD).Methods A retrospective study was performed to collect date of 114 COPD patients in the Hospital from January 2021 to January 2022.Pulmonary Arterial Systolic Pressure(PASP)was determined.Among them,74 patients had PASP.COPD patients with pulmonary hypertension were divided into two groups as the diagnostic criteria:36mm Hg≦PASP≦50mm Hg mild pulmonary hypertension,50 mm Hg < PASP moderate and severe pulmonary hypertension.In addition,there were 40 healthy controls.General clinical data(age,gender,smoking status,disease history)and echocardiographic indicators of each group were summarized.Mean Platelet Volume(MPV),Red Cell Distribution Width(Red Cell Distribution Width,RDW),White Blood Cell(WBC),Platelet Count(PLT),and Platelet Distribution Width,Hemoglobin(HGB),Neutrophil(NEU),and serum N-terminal cerebral natriuretic peptide(NTproBNP).The differences in general conditions,echocardiographic indicators and serology of each group were statistically analyzed.Univariate logistic regression analysis and multivariate logistic regression analysis were used for information classification.The independent risk factors of COPD were compared with ph value,and the correlation between risk factors and PAP.ROC curve was used to evaluate the diagnostic value of risk factors.A p value of less than 0.05 was considered to indicate statistical significance,and an area under the curve(AUC)of more than 0.6 was considered to indicate strong diagnostic power.Results(1)There were statistically significant differences in RDW,MPV and NTproBNP among the three groups(P < 0.05).Pairwise comparison showed that RDW,MPV and NT-proBNP in the mild pulmonary hypertension group and the moderate and severe pulmonary hypertension group were significantly higher than those in the normal control group(P < 0.05).(2)RDW,MPV and NT-proBNP in the moderate to severe pulmonary hypertension group were significantly higher than those in the mild pulmonary hypertension group(P< 0.05).(3)Univariate Logistic regression showed that age,HB,HCT,RDW,MPV,D-dimer,NT-proBNP,and PCT were identified as variables associated with the risk of COPD combined with PH.Further multivariate Logistic regression analysis showed that RDW,MPV and NT-proBNP were independent risk factors for COPD patients with PH(P<0.05).(4)In the pulmonary arterial hypertension group,correlation analysis of RDW,MPV,NT-proBNP and PASP showed that RDW,MPV,NT-proBNP were positively correlated with PASPConclusions RDW,MPV,and NT-proBNP were significantly increased in COPD patients with PH and thus may be useful biomarkers for PH secondary to COPD.
